1.0 PURPOSE

This procedure describes the mandatory methodology for cutting, preparing, packaging, delivery and documentation for destructive test samples of mine hoist ropes.

2.0 SCOPE

This procedure is applicable to all mines required by regulation to submit samples of mine hoist ropes for destructive tests. This applies to all drum type hoists.

2.1. EXCEPTIONS

In-service destructive tests of Koepe hoist ropes are not required by regulation and are therefore and exception from this procedure.

3.0 REFERENCES

The following references were used in the development of this document or are related to it. They shall be used in their most recent revision.

CSA G4-09 – Steel wire rope for general purpose and for mine hoisting and mine haulage

O. Reg 854 – Occupational Health and Safety Regulation for Mines and Mining Plants

4.0 DEFINITIONS

MOL-MTL: Ministry of Labour – Materials Testing Laboratory

5.0 PROCEDURES

5.1. PERSONNEL

A person shall be designated at each mine site responsible to ensure this procedure and

processes are followed for all mine hoist rope destructive test samples. In most cases this is the

fixed plant maintenance planner. The designated person also manages the Rope Record Book.

Training required is Rope Record Book Training SSF098.4.

5.2. SAMPLE CUTTING

Sample cutting, clamping and handling as per “Recap Instructions”, see Appendix A.

a. Keep sample as straight as possible.

b. Put all clamps on rope prior to cutting.

c. Electric punch-lock tool may be used.

d. Recommended to use a pre-marked 8’ long stick as a template for locating the clamps.

e. Abrasive disc cutting only.

f. Place the metal tag with rope reel # in the middle of the 8’ sample.

5.3. DOCUMENTATION

Shipping labels and warehouse shipping form shall be used, see Appendix B, C. Labels and

forms to be pre-printed and plastic laminated prior to the rope cutting job.

a. Two shipping labels:

1 copy on shipping tube,

1 copy on rope.

b. Three warehouse shipping forms:

1 copy goes to the lab, signed by delivery person and the lab attendant and is left

with the lab,

1 copy goes to the lab, signed by delivery person and the lab attendant and comes

back to the mine to be handed back to the Contact Person and is kept in the rope

file

1 copy is kept at the mine, overwritten “sample copy”. Brought back to Contact

Person after sample has been packaged and put on the delivery truck.

c. Provide a metal tag, stamped with the rope reel number, wired to the middle of the 8’

sample. This tag will remain on the sample from the time it is cut until it is destructively

tested at the MOL-MTL.

5.4. SAMPLE CHAIN OF CUSTODY

Procedures for chain-of-custody handling of the sample from the collar floor to MOL-MTL:

a. Shipping label and metal tag attached to sample as it is cut and banded and placed into

the labeled shipping tube. Make sure the labels match the metal reel # tag. This is to be

done at collar as the rope sample is cut.

b. Sample goes directly from collar to designated area, and then delivered to MOL-MTL as

soon as possible by the Contact Person or designate. Lab will receive samples weekdays

between 7:30am and 4:00pm weekdays. If sample cannot be delivered within the

available hours, wait for the next available day.

c. Delivery person and MOL-MTL technician both verify sample has been properly labeled

and received; both persons sign the two copies of the warehouse form. If for some

reason this process can’t be completed, the delivery person is to go back to the mine with

the sample and return with the sample on the next available day.

5.5. RECEIPT OF TEST RESULTS

a. Test report is emailed by MOL-MTL to Contact Person and to Hoisting Group

([email protected]).

b. For any test results that are at “removal” stage, MOL-MTL will inform the Contact

Person by telephone.

c. Contact Person and Hoisting Group verify test report information:

Rope reel #

Initial test #

Shaft, compartment

Rope diameter, wire diameters

Proper initial test data used for comparison

Review results for any anomalies:

Removals

Cautionaries

Any significant change from previous test results

5.6. RECORDING AND STORAGE OF TEST RESULTS

a. Mine Contact Person records test data in the Rope Record Book.

b. Hoisting Group keeps all test reports on a network folder accessible on Mines

Hoisting webpage.

c. Contact Person keeps the documentation as:

Hard copy in binder of current rope documentation

Electronic copy in equipment folder on network

5.7. PROCUREMENT AND PAYMENT OF TEST SERVICES

a. A Purchase Order (PO) is created by each mine for several months’ worth of testing.

b. PO number is noted on the sample label and shipping form.

c. MOL-MTL will not test rope unless there is a PO number provided.

d. MOL-MTL emails their “ARIR” form with the test report, this is the “pro-forma invoice”

that verifies to the Releaser that the work has been completed.

e. Contact Person forwards email with ARIR form to Releaser.

f. Releaser creates and emails SES# to MOL-MTL (cc to contact person) so that MOL-

MTL can include this information on the invoice to expedite payment.

g. MOL-MTL sends invoice to government Payment Processing Centre (PPC). PPC

mails invoice to Vale Shared Services Organization (SSO). SSO processes the

invoice using the SES# provided.

ROPE SAMPLE INFORMATION

1. The eight foot sample should be properly seized, tagged and transported preferably in a plastic tub of appropriate size. (Refer CSA G4-09 Section 15.3.1 for sampling procedures for new ropes at manufacturer’s plant and 15.3.2 for sampling procedure at mine site.)
