C. Pre-Enrollment Procedure

1. Accomplish Entrance Application Form (EM) - Senior High School for examination 2. Submit accomplished Entrance Applicaticm Form (EM) - Senior High School, Photocopy of IRS. Report Card to the Adniissions,

Scholarships and Placement Office. (ASPO) 3. Secure payment slip from ASPO. 4. Pay entrance examination / pre-registration and get receipt of payment at the USA Accounting Department/Cashier 5. Present payment receipt to the ASPO and get the entrance test schedule. 6. Take Entrance Examination as scheduled at the Guidance Services and Testing Center (GSTC). 7. Get entrance test result from the ASPO. S. Get Interview schedule from the Senior High School Office. 9. Submit oneself for interview.
