• Sustainability Steering group oversees progress of SDMP and SDAP
• Agree dedicated resource to deliver sustainability• Building partnerships to deliver effective early interventions,
support community recovery and reduce readmissions• Consider the use of technology as patient empowerment tools• Mobilise external organisations to deliver improvements to
support the Trust’s aims• Consider future service changes (demographics) or adaptation
needs (resilience)
