ENDORSEMENT OF NAME OF WIFE FOR FAMILY PENSION IN PP

bf To avoid hardship to the family, in the event of death of the service pensioner, in 1988, insy Defence had issued detailed instructions for endorsement of family pension entitlement in the Pans Payment Order of living pensioners. These instructions were well published and publicised for the bene

of pensioners and their families.

However, yet it is observed that still a large number of Service Pensioners retired prior to 1986 have 2. not applied for endorsement of family pension in Pension Payment Order (PPO). In number of cases, it has been found that the service pensioner had married after his discharge from service, but they have not applied for promulgation of NE POR TOr marriage and endorsement of his wife's name in service records as

Next of Kin. In some cases, pensioners have re-married after the death/divorce of his 1st wife, but not applied for promulgation of NE POR Tor death/divorce of 1st wife and re-marriage. Similarly, pensioner has not applied for promulgation of NE FOR TOr the child born after his discharge of service.

3 It is pertinent to mention that in the event of death of Service Pensioner, it becomes very difficult and takes a considerable time for authorisation of family pension to the entitled family member, in the absence

of name of family members endorsed in service records. It is not out of place to reiterate that our objective is to facilitate unhindered grant of entitled facilities to our Air Veteran and their families

It is advised to all Service Pensioners whose wife's name is neither notified in Original PPO nor notified through Corrigendum PPO/ JN PPO, is to immediately apply for endorsement of wife's name in PPO. It may be noted that name of wife mentioned in Pension Certificate/Book as Next of Kin only in respect of pre-1986 retiree is not to be treated as wife's name notified for Pension.

The following documents / information are required for endorsement of family pension in PPO, if wife's name is not endorsed earlier in PPO in case of pre-86 retiree or marriage / re-marriage after discharge from service.-

(a) Appendix-A (Application for Endorsement of Family Pension), in triplicate, (All three copies should be signed in ink by the Pensioner, his Spouse & Witnesses in Part-I and Pension Disbursing Agency (PDA) i.e. DPDO/ Bank/ Treasury from where drawing pension in Part-ll.

(b) Annexure-l1, in triplicate. (All three copy should be signed ink by the Pensioner in Part-I and Pension Disbursing Agency (PDA) i.e. DPDO / Bank / Treasury from where drawing pension in Part-ll

(c) All columns of Appendix-A and Annexure-ll must be filled in ink, Joint Photographs to be pasted on Appendix-A and attested by any gazetted officer. Part-ll of the Appendix-A and Annexure-lI (all three copies) to be completed by your Pension Disbursing Agency (i.e. Bank/DPDO/TO etc) with their office stamp. Fill up all the columns of the forms carefully to avoid rejection at later stage. Please ensure that the name of wife is written same as mentioned in your Pension Certificate/Book issued after discharge from service and the same name including spelling should be there in your Aadhar Card & PAN Card of wife). In case of mismatch of word or ietters, you have to apply for change of name first

(d) Attach Spouse's date of birth proof in which date of birth is mentioned in the format of Date, Month and Year DD/MM/YYYY (i.e. Birth Certificate / Matriculation certificate / PAN card / Aadhar Card / Passport etc). It may be noted that the name and date of birth should match with

service documentsI PPO / Pension Certificate. In case applied for change of name of wife, changed named should match with the name in DOB proof (Aadhar Card/PAN Card)

(e) Full Details (Name & Address) of Pension Disbursing Authority (ie DPDO/Bank/TO) is to be

attached. If PDA is Bank, pension account number is also to be mentioned. Please submit PDA details proforma along with application. (f you are submitting Annual Life Certificate to DPDo and they are sending pension to your Bank a/c, your PDA will remain DPDO only and not the

Bank).

() at the time of discharge from Service, duly attested by Pension Disbursing Authority / Zila Sainik Board is also to be attached.
