Student Plans EMERGENCY CARE PLAN Even though the emergency care plan is found on the Health Plan menu by default, an Individual Health Plan does not have to exist in order to create an emergency care plan. To create an Emergency Care Plan, the case manager or nurse must have rights to be a Health Plan Manager. Students must be assigned in the Assign Cases section to that individual, as a Health Plan Manager. After an Emergency Care Plan is created and is in either Final, Correct or Inactive status; paras, transportation staff or teachers can receive a printed copy of the plan using the student summary report. EMERGENCY CARE PLAN

Even though the emergency care plan is found on the Health Plan menu by default, an Individual Health Plan does not have to exist in order to create an emergency care plan. To create an Emergency Care Plan, the case manager or nurse must have rights to be a Health Plan Manager. Students must be assigned in the Assign Cases section to that individual, as a Health Plan Manager.

After an Emergency Care Plan is created and is in either Final, Correct or Inactive status; paras, transportation staff or teachers can receive a printed copy of the plan using the student summary report. The student summary may be printed in batch mode or individually by student. (Refer to Student Summary Reports in Reporting section of the documentation.)

Creating an Emergency Care Plan

1. As Health Plan Manager, click on My Cases

Note that the column ‘Last IHP’ is blank for these students. Also, each column is sortable. Click on column heading to sort.

2. Click on View Health Plans link to view active ECPs or IHPs or both

Note that the health manager assigned to these cases is displayed

3. Select the school to view. Click submit. Both active IHPs and ECPs are displayed as the default.

4. Select student. Select Health in the left menu. This will only display if you have activated this in menu customization.

Click on Create New Emergency Care Plan link.

5. Input start date of ECP. Click submit. By default, the start date is today’s system date. If this plan is for a limited amount of time (ie. Broken leg, recovery from surgery), input the stop date. Otherwise, this field is may be left blank. NOTE: Only one care plan is allowed for any given date.

6. The assessment screen will be displayed.Input assessment and click submit.

Assessment is the School Nurse’s assessment of the child’s health condition.

7. Click on Symptoms link on Emergency Care Plan menu.

Symptoms/actions can be added manually or via the Nursing Library. If a symptom is deleted from the plan, all actions associated with it will also be deleted.

8. Click on Add New Symptom link Fill in information

9. Click Submit and View Actions button. Click Add New Action

Input actions that staff should DO or NOT DO if this symptom occurs

10. Key in one action per screen. Select whether this action is a DO or a DON’T

Normally, input the highest priority actions first. Actions can be resequenced by changing the sequence number. If necessary, click spellcheck after data is submitted.

11. ‘Emergency Action was Successfully Added’ message will be returned

Add symptoms/actions for student from Nursing Library

12. Select add new symptom from library Symptoms/actions for nursing areas are only available if they have been previously setup in district maintenance by your system administrator. Also, some symptom/actions may have been setup with an auto include option. If the symptom/action is setup with an auto include flag it will be automatically included into the ECP

13. Search library for symptom. Click submit. In this case we want to search for Asthma

14. Select symptoms for this emergency care plan by clicking in the box to the left of the symptom

Multiple symptoms may be selected if available. Actions will then be added for each symptom selected

15. Click actions link for symptom added from library

16. Click Add New Action from Library link

17. Select the actions required for the symptom of Asthma Anxiety. Click submit.

18. A message that the actions were successfully entered should be displayed.

If desired, the actions can be resequenced by clicking in the sequence field, changing the sequence number. Click ‘submit sequence changes’ button for changes to

take effect.

19. Click Notes link on Emergency Care Plan menu

Notes are optional and not a required section. The system default is to not print the notes with the ECP, but this can be changed in document sections. Notes are intended to be used by nurses to communicate between each other.

Print Emergency Care Plan20. Click Print Emergency Care Plan on the

menu, to ensure that all the symptoms and actions have been input correctly

This will display the document. It will not print unless print is selected on the PDF menu. The document will read as DRAFT until it is finalized with completeness check.

21. Click check completeness on the Emergency Care Plan menu

After document has been reviewed and all items are complete, click on Make Final button

22. Once an Emergency Care Plan is in final status, it may be copied for next school year and edited accordingly

It is recommended that there only be one active Emergency Care Plan at one time. As with all plan documents, the start and stop dates cannot overlap

Change Final ECP to Not Implemented Status23. Click on Final link. Select Not

Implemented. Click SubmitA finalized ECP cannot be deleted but it can be changed to Not Implemented

24 Key in the status reason. Click submit This ECP will become inactive and not display on View Health Plans or on the tickler list

Print Individual Student Summary Report25. Click Print Student Summary on main

menu for studentIn our example, a student summary report called ECP was setup previously by the system administrator. If there are no reports displayed in the drop down menu they will need to be setup.

26. Select ECP report from drop down menu. Click submit

27. Based on the report specifications in the system/district maintenance document sections, only the elements that have the options ‘Print Always’ or the ‘Print if Data is Present’ will print
