BackgroundDentinehypersensitivity(DH)isacommonoralpaincondition,arisingwhendentineisexposedbytoothwearorgingivalreces-sion,arecentsystematicreviewreportinganaverageprevalenceof42%.Ashort,sharp,arrestingpainisexperiencedwhenpat-entdentinetubulesareexposedtostimulisuchascoldfoodanddrink,elicitingthemovementoffluidwithinthetubulesandtrig-geringapulpalnerveresponseanddentalpain.AlthoughDHpainisusuallytransientitcanbeintenseandhasbeenshowntohaveanegativeimpactonqualityoflife.Treatmentactstodepolarisethenervesandorblockthedentinetubules,beingprofes-sionallyadministeredoroverthecounter.Overthecountertreatmentareconvenientandsimpletousehoweverandmanyfor-mulationsshowefficacycomparedtonegativecontrols,howeverevidencefromsystematicreviewsshowsthereisno‘goldstan-dard’ treatment.Thisstudyaimedtotesttheefficacyofanexperimental toothpastecontainingpotassiumnitrate,aluminiumlactateandhydroxyapatitetoreduceDHpainascomparedtoapotassiumnitratecontroltoothpaste.MethodsThiswasarandomised,examinerblind,twotreatmentarm,parallel,stratified,controlledtrialinadultparticipantswithDH.Aftera7-daywash-inperiodparticipantswith2sensitiveteeth(Schiff>2)wererandomisedtotestorcontroltoothpaste.DHofthese-lectedstudyteethwasassessedbytactilestimuliwithaYeapleprobe,Schiffscorefollowingadropofwaterat0°CandVASonglobalDH,atbaselinepreandpostsupervisedbrushingandafter7and14daysofproductuse.DHqualityof lifeanda fullmouthTureskyplaquescorewerealsocompletedatthesevisits.Results82participantswererandomisedandcompletedthestudy.TherewasanimprovementinDHpainscoresinbothgroups,butim-provementsweremuchgreaterinthosereceivingthetesttoothpastewithpainreducedsignificantlymorethanthoseusingthecontroltoothpasteatalltimepointsforallDHmeasures(p=0.005).Furthermore,usingthetestasopposedtothecontroltooth-pasteresultedinasignificantrelativeriskreduction(RRR)ofDHpainof55%immediatelypostsupervisedtoothbrushingrisingto81%after7and88.6%after14days(p<0.001).Meanqualityoflifescoresimprovedslightlyoverallfrombaselinetovisit4inbothgroups,however,therewerenosignificantdifferencesbetweenthemcomparingtheoverallscoreorforanyindividualques-tions.Similarly,therewasnosignificantdifferenceinglobalVASscorereportedbytheparticipantsinthetestandcontrolgroupsalthoughscoresfavouredthetesttoothpaste.Plaquescoreswerealsosimilarforbothtoothpastesatalltimepoints.ConclusionTheexperimentaltoothpastecontainingpotassiumnitrate,aluminiumlactateandhydroxyapatiteiseffectiveatreducingDHpainimmediatelyaftertreatmentandforupto14days,providingsignificantlymorepainreliefthanthebenchmarkcontrolpotassiumnitratecontroltoothpaste.
