認知機能の客観評価と認知症早期発見の新方法・実用化技術

認知症（Dementia）は，後天的な脳の器質的障害により，いったん正常に発達した知能が低下した状態をいう．中核症状は記憶障害と認知機能障害（失語・失認・失行・実行機能障害）から成る．

認知症の種類

認知症は、大きく２つに分けられる．1. 「アルツハイマー型認知症（AD,Alzheimer’s Disease）」

発症原因は脳内にあるアミロイドβ蛋白(Aβ) が脳内で蓄積することで，神経細胞死を来たし，脳機能が低下し，認知症を来たすと考えられている．

2. 「脳血管性認知症（Vascular Dementia）」等

脳梗塞や脳卒中，脳出血など，脳や脳の血管にまつわる病気が原因で引き起こされる．

認知症現状

日本では2012年に認知症患者４６２万人（65歳以上の15％、85歳以上は40％）、軽度認知症障害（認知症予備群４００万人）

世界では、現在3560万人、2030年に6570万人、2050年には1億1540万人に増加する

臨床診断の現状

臨床的に主に使用されている方法として，アンケート形式の認知機能問診テスト（MMSEなど）が挙げられる．

認知症の臨床診断法

認知機能問診テストの問題点担当医師から数項目の問題を提示し，その回答に

よって点数が付けられる．しかし，問診項目として「知っている野菜の名前をできるだけ多く言ってください」などの内容があり，個人生活環境によって検査結果は異なることがある．従って，従来の認知機能テストは客観性が欠かせている．

例えば，「今日は何年何月何日何曜日ですか？」という質問に対して，患者のみならず，健常者でも忘れることがよくあり，客観的な検査ができない．

Results – Response time and Accuracy-

• Our results suggest that the mean RT of patients with AD was significantly longer than those of patients with MCI and NC individuals.

• The mean accuracy of the AD group was significantly lower than those of the MCI and NC groups under auditory selective attention with A_stimuli and in the audiovisual divided attention session with A_stimuli.

• Moreover, under both divided and visual selective attention conditions, the RTs for the bimodal AV_stimuli were significantly fasterthan the RTs for the unimodal V_stimuli or A_stimuli.

• CDF allows for a comparison of the multisensory condition with the joint probability of the unimodal conditions. – CDFs: (P[A] + P[V]) − (P[A] × P[V]). P(A) is the probability of the A_stimuli tria, and P(V) is the

Results -Performance Enhancements-

• This table represents the statistical significance of a one-sample t-test at each time bin within each group to compare the CDF curves to zero.

• The gray and black blocks show the different statistical significance values.

• The * represents the time bin in which peak behavioral enhancements were observed for each subject group.

• The onset time and peak benefit of audiovisual integration in MCI and AD patients occurred significantly later than did those of the NC.

触覚認知の脳内活動

• 触覚は人間にとって，重要な役割を果たす．特に人間の指先は繊細であり，指先を用いて物の形や材質などを弁別する場合は多くの脳領域の賦活が必要となる．

触覚形状刺激の選択

• 当研究室は，認知科学実験と機能的磁気共鳴画像（fMRI）

などの手法を用いて，触覚による認知症早期診断の新たな研究アプローチを提案した．

• 実物模型或いは単純な幾何学形状を触覚刺激としたことが多く，視覚的な経験などの情報が含まれている．

触覚角度刺激の選定

• 二次元の触覚形状は，図形の線分の長さ・曲率と線分間の角度の三要素によって構成されている

• 我々は，触覚図形を構成する重要な要素の一つである「角度」に注目し，健常若者を用いた指先触覚による角度弁別（２つの異なる角度を区別する）特性を解明した.

実験結果

指先触覚による角度弁別正答率はMCIとAD患者ともに健常高齢者より低いことを明らかにした．

また，角度弁別閾値はAD患者が最も大きく，健常高齢者に最小であることを明らかにした．

さらに，MCI患者とAD患者の間にも有意な差が得られた．

想定される用途

• 本技術は、認知症を客観的に早期発見することで、病院、医療福祉施設で認知症の早期診断応用にメリットが大きいと考えられる。

• 上記以外に、老人施設、家庭での使用により認知予防などの効果が得られることも期待される。

• また、達成された早期発見に着目すると、早期治療といった分野や用途に展開することも可能と思われる。

実用化に向けた課題

• 現在、認知脳機能の基礎研究成果に基づいて認知症早期発見が可能なところまで研究開発済み。しかし、検査方法の簡易化、検査装置の便利化・コストダウン、検査結果のネットワーク化などの点が未解決である。

• 今後、検査の方法・装置・結果処理について実験データを取得し、実用化に向けて研究開発を推進していく。

企業への期待

• 未解決の検査方法の簡易化、検査装置の便利化・コストダウン、検査結果のネットワーク化については、製造またはIT技術により克服できると考えている。

• 製造またはIT技術を持つ、企業との共同研究を希望している。

• また、高齢化社会用品を開発中の企業、健康・医療福祉の分野への展開を考えている企業には、本技術の導入が有効と思われる。
