Diabetes is becoming a pandemic
The number of people with type 2 diabetes is increasing in every
country
80% of people with diabetes live in low- and middle-income
countries
The greatest number of people with diabetes are between 40 to
59 years of age
183 million people (50%) with diabetes are undiagnosed
Diabetes caused 4.6 million deaths in 2011
78,000 children develop type 1 diabetes every year
![Page 3: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/3.jpg)
Diabetes in South East Asia
Close to one-fifth of all adults with diabetes in the world live in the South-East Asia Region
The region has the second highest number of deaths attributable to diabetes of any of the seven IDF regions with 1.16 million deaths in 2011
![Page 4: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/4.jpg)
Obesity and Diabetes
Obesity and physical inactivity are well-established risk factors
for the development of type 2 diabetes
It is estimated that for every 1-kg increase in weight, the
prevalence of diabetes increases by 9%
Physical inactivity is associated with increased insulin
resistance and poorer glycemic control independent of body
weight
Just 39% of adults with diabetes are physically active
compared with 58% of other American adults
Modest weight loss through diet and physical activity reduces
the incidence of type 2 diabetes in high-risk individuals by
∼40–60% over 3–4 years

ACUTE EFFECTS OF EXERCISE
Insulin-independent and insulin-dependent muscle glucose uptake during exercise.
At rest and postprandially, its uptake by muscle is insulin dependent and serves primarily to replenish muscle glycogen stores.
During exercise, contractions increase BG uptake to supplement intramuscular glycogenolysis
![Page 13: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/13.jpg)
ROLE OF GLUT 4
Insulin activates GLUT4 translocation through a
complex signaling cascade .
Contractions, however, trigger GLUT4 translocation
at least in part through activation of 5′-AMP–
activated protein kinase.
Insulin-stimulated GLUT4 translocation is generally
impaired in type 2 diabetes .
Both aerobic and resistance exercises increase
GLUT4 abundance and BG uptake, even in the
presence of type 2 diabetes.
![Page 14: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/14.jpg)
Glucose transport into skeletal muscle
![Page 15: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/15.jpg)
Glucose uptake Skeletal muscle
Insulin-stimulated BG uptake into skeletal muscle
predominates at rest and is impaired in type
2 diabetes
Muscular contractions stimulate BG transport via a
separate additive mechanism not impaired by
insulin resistance or type 2 diabetes.
![Page 16: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/16.jpg)

Effects Of Aerobic Exercise
In nondiabetic persons :
Peripheral glucose uptake = Hepatic glucose output
Blood glucose does not change
Plasma insulin levels normally fall
![Page 19: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/19.jpg)
Effects Of Aerobic Exercise
In type 2 diabetes
BG utilization by muscles > hepatic glucose output
BG levels tend to decline
A single session increases insulin action and glucose tolerance for more than 24 h but less than 72 h
During brief, intense aerobic exercise, plasma catecholamine levels rise markedly
Hyperglycemia can result from such activity and persist for up to 1–2 h
![Page 20: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/20.jpg)
Effects of Resistance exercise
In individuals with IFG resistance exercise results in lower fasting BG levels 24 h after exercise
![Page 21: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/21.jpg)
Combined aerobic and
resistance training
A combination of aerobic and resistance training may be
more effective for BG management than either type
of exercise alone
• Resistance training:
An increase in muscle mass that may contribute to BG
uptake without altering the muscle's intrinsic capacity to
respond to insulin
• Aerobic exercise :
Enhances its uptake via a greater insulin action,
independent of changes in muscle mass or aerobic
capacity.
![Page 22: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/22.jpg)
Acute changes in insulin resistance
• The acute effects of a recent bout of exercise account for most of the improvements in insulin action, with most individuals experiencing a decrease in their BG levels during mild- and moderate-intensity exercise and for
2–72 h afterward
BG reductions are related to
duration of the exercise intensity of the exercise pre exercise control state of physical training
![Page 23: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/23.jpg)

Acute changes in Hepatic Glucose
Handling
Hepatic lipid content (HLC)
associated with obesity and type 2 diabetes leading
to reduced hepatic and peripheral insulin action.
![Page 26: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/26.jpg)
Chronic Effects Of Exercise Training
• From aerobic exercise
Even 1 week of aerobic training can improve whole-
body insulin sensitivity in type 2 diabetes
Moderate training may increase glycogen synthase
activity and GLUT4 protein expression but not
insulin signaling
Type 2 diabetes may be associated with a decrease
in lipid oxidation and shift toward greater
carbohydrate oxidation at all exercise intensities
![Page 27: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/27.jpg)
Chronic Effects Of Exercise Training
• Resistance exercise training
Increase in muscle mass- BG uptake because of mass
effect
Reversal of skeletal muscle loss or prevent further
due to disuse and aging
In a randomized controlled trial (RCT), twice-weekly progressive resistance training for 16 weeks by older men with newly diagnosed type 2 diabetes resulted in a 46.3% increase in insulin action, a 7.1% reduction in fasting BG levels, and significant loss of visceral fat
![Page 28: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/28.jpg)
Lipids and lipoproteins
Aerobic training
Decreases total and LDL cholesterol
Raises HDL cholesterol
• A meta-analysis of training effects on blood lipids in adults with type 2 diabetes found, that LDL cholesterol may be reduced by ∼5%
• Lipid profiles benefit more from concomitant exercise training and
weight reduction
• Most lifestyle interventions have been accompanied by an approximate 5-kg weight loss
![Page 29: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/29.jpg)
Hypertension
Both aerobic and resistance training can lower BP in
nondiabetic individuals
Systolic BP : Reductions of 4–8 mmHg
Diastolic BP: Reductions less common
![Page 30: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/30.jpg)
Mortality and CV risk
All-cause and CV mortality risk was 1.7–6.6 times
higher in low-fit compared with high-fit men with
type 2 diabetes
Fittest men lowest risk
![Page 31: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/31.jpg)
Body weight: Maintenance and Loss
Long-term weight control :
Diet + Exercise + Behavior modification
• Exercise interventions undertaken with volumes typically
recommended to improve BG control and reduce CVD risk
(e.g., 150 min/week of brisk walking) are usually insufficient for major weight loss
• About 1 h of daily moderate aerobic exercise produces at least as much fat loss as equivalent caloric restriction, with resultant greater insulin action.
![Page 32: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/32.jpg)
Psychological effects
Look AHEAD trial:
Participants in the intensive lifestyle intervention attempted to lose more than 7% of their initial weight and increase moderately intense PA to greater than 175 min/week
Physical fitness in type 2 diabetes
Reduced Depression
Improve health-related QOL in those with
![Page 33: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/33.jpg)
PRE EXERCISE EVALUATION
• Exercise stress testing before walking is unnecessary
• Exercise more vigorous than brisk walking or exceeding the
demands of everyday living should
undergo graded exercise test
• Stress testing should not be routinely recommended to
detect ischemia in asymptomatic individuals with a low CAD
risk (<10% risk of an event more than 10 years)
The lower the CAD risk, the higher the chance of a false positive
![Page 34: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/34.jpg)
ECG stress testing
Indicated for individuals matching one or more of these criteria:
• Age >40 years, with or without CVD risk factors other than diabetes
• Age >30 years and
– Type 1 or type 2 diabetes of >10 years in duration
– Hypertension
– Cigarette smoking
– Dyslipidemia
– Proliferative or pre proliferative retinopathy
– Nephropathy including microalbuminuria
• Any of the following, regardless of age
– Known or suspected CAD, cerebrovascular disease, and/or peripheral artery
disease (PAD)
– Autonomic neuropathy
– Advanced nephropathy with renal failure
![Page 35: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/35.jpg)
Aerobic exercise training
Resistance exercise training
• Frequency: 3 days/week with no more than
2 consecutive days between bouts of activity
• Intensity: Moderate intensity, 40–60% of Vo2max e.g. brisk walking
• Duration: Minimum of 150 min/week moderate intensity, for example walking at 6.4km /h , each bout of at least 10 min (30 min, 5 days/week) or 60 min of vigorous PA (20 min on 3 days)
• Mode: Any form of aerobic exercise (including brisk walking) that uses large muscle groups
• Frequency: at least twice weekly on nonconsecutive days
• Intensity: Moderate, (50% of 1-repetition maximum [1-RM]) or vigorous (75–80% of 1-RM)
• Duration: Each session should minimally include 5–10 exercises involving the major muscle groups and involve completion of 10–15 repetitions to near fatigue per set., but as many as three to four sets.
• Mode: Resistance machines and free weights (e.g., dumbbells and barbells) can result in fairly equivalent gains in strength and mass of targeted muscles
![Page 36: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/36.jpg)
Intensity of aerobic exercise
“Moderate”
40–60% of V̇O2max
(∼50–70% of maximum heart
rate)
“Vigorous”
>60% of V̇O2max
(>70% of maximum heart rate).
MAXIMUM HEART RATE = 220 MINUS AGE
Example for 40 year old person
Moderate will be with target HR = 90 - 125/min
![Page 37: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/37.jpg)
MET (metabolic equivalent)
It is a unit of intensity equal to energy expenditure
at rest.
Physical activity at 3 METs uses three times as much
energy as stationary sitting.
![Page 38: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/38.jpg)
Daily movement
(unstructured activity)
• Nonexercise activity thermogenesis Energy expending for activities of daily living
It can create a large daily caloric deficit to prevent
excessive weight gain
• Most of the lean subjects' greater activity came
from walks of short duration (<15 min) and low
velocity (∼1 mph)
• Meta-analysis of 26 studies with a total of 2,767 (primarily nondiabetic) participants found that pedometer users increased PA by 26.9% over baseline in studies having an average intervention of 18 weeks.
![Page 39: Dr. Syed Khurram Shahid Consultant EndocrinologistDiabetes is becoming a pandemic The number of people with type 2 diabetes is increasing in every country 80% of people with diabetes](https://reader033.vdocuments.site/reader033/viewer/2022060709/6074ed103c2b0b009a4a0d3d/html5/thumbnails/39.jpg)
Exercise With Non optimal
Glucose Control
• Hyperglycemia:
No need to postpone exercise because of high BG
• Hypoglycemia:
Very intense activities can cause transient elevations
in BG
Controlled by lifestyle alone: risk of hypoglycemia is minimal
Insulin or secretagogue users: Pre exercise BG levels of <100
mg/dl 5–30 g of carbohydrate should be ingested before any PA
