1. Log on to https://www.unitedhealthproducers.com/

2. Enter Username & Password

3. Click on Product Information & Materials

4. Click on MATERIALS, on the left hand screen

5. Click on Sales Materials

6. Select Location: State & County and Click SEARCH

(The Materials Search engine allows you to search by location or by the item number if known)

7. Select Plan Materials

8. Enter QTY

9. Select ADD TO CART
