GENERAL REMINDERS/INSTRUCTIONS FOR NSTP 2 SUPERVISING INSTRUCTORS:
1. Calendar of NSTP 2 for the coming days:a. ____________final touches with apostolate teams on all the requirements.b. ____________extension of final touches, processing of remedial due to deficiencies and demerits,
and processing of clearances 2. Ensure that your students have properly accomplished the apostolate portfolio vis-à-vis content
and format. NSTP Coordinator will not accept portfolios that did not follow said content and format. (see format below)
3. Ensure that your apostolate teams who have been borrowing materials from the NSTP office have returned all the said materials
4. Ensure that the form 1.2 of the students is properly accomplished: 5. Administer your post exams. The total points should be _________. The post exam is primarily
focused on assessing the involvement of the students in the actual procedures and activities of the apostolate. SI can expound the coverage as may be necessary
6. Administer your post conference: review with your students the progress of your apostolate from the enrollment of nstp 2, the processing of the clerical requirements to the actual apostolate implementation. Highlight on significant events (these are usually the “items” that exhausted the energy, attention, time, or concentration of the students; items that may be unusual to the students, positive or negative, strengths and weakness experienced. Analyzed with the students why these things happened and explain their implications. Present their apostolate experience as a possible pre-cursor to the real work place or life our students will be immersed in come their “life after NSTP.”
7. Before clearance with the NSTP Coordinator, please make sure general requirements are properly accomplished and prepared. Clearance is given ONLY to apostolates/students who have substantially accomplished the apostolate as defined by the objectives and activities in the concept paper. As specified above and as deemed necessary, NSTP coord will inspect first final results of apostolate before clearance. Please do not assure your students that they will get cleared without the NSTP Coordinator giving the go signal or him checking the general apostolate outputs.
8. Emphasize to your students NO GOING HOME UNLESS CLEARED. This is to avoid the instances where should there be remedials or revisions in the apostolate requirements or other accountabilities, only the leader is around to be fixing things.
9. Should your students need remedials due to demerits and/or deficiencies, refer them immediately to our provisions and guidelines on undergoing remedial as posted and as herein attached. Please do not give remedial not approved by the NSTP Coordinator. While there are lists of remedials prepared already in the office, please feel free to suggest and discuss your suggestion on remedials to the NSTP Coordinator.
10. Make sure you have acted on/ implemented endorsements of concerned students and apostolates as endorsed to you by the Coordinator through the letter of the concerned student/ apostolate and/or through the direct endorsement slip of the NSTP Coordinator.

LIST OF REQUIREMENTS
INDIVIDUAL:
1. Form 1.2: c/o SI 2. Enlistment in an apostolate/project3. Original Copy of the General Guidelines of NSTP 2: submit to the SI and filed in the group
folder; conforme slip submitted earlier to SI4. Individual journal (Individual report on orientation, consultations, meetings and apostolate
proper): properly accomplished, submit to the SI and filed in the group folder5. Individual copy of the approved apostolate file (procedure, approval sheet, concept paper,
activity papers, general instructions for apostolate success and safety precautions/do or don’t’s, waiver)
6. Actual Apostolate participation: recorded in the form 1.27. Culmination Activity/Post conference attendance: recorded in the form 1.2 8. Team Performance Evaluation: to be facilitated by respective Supervising Instructors (SI’s are
instructed to ensure confidentiality of the process and result of the evaluation): results filed in the folder and recorded in the form 1.2

in the form 1.2; test questions, exam/activity papers are filed in the group folder 10. test permit for finals: soon as available, student proceeds to the NSTP Office and have it
recorded in the form 1.211. Any available merits: recorded in the form 1.212. NO remedials: cleared from excess demerits (violations of behavioral guidelines and other
instructions) and deficiencies (absences and tardiness): recorded in the form 1.213. Clearance from (use the clearance form prepared by the NSTP Office. Clearance is secured
from: a. Apostolate Team Leaderb. Host Community/agency/ Officer-in charge if it appliesc. Supervising Instructord. NSTP Office Admin Assistante. NSTP Coordinator: clearance by the coordinator is given only when apostolate is
substantially completed (if apostolate objectives and activities as defined in the approved concept paper are substantially followed/implemented) and when the apostolate portfolio is properly accomplished by the team and duly endorsed by the SI and submitted to the coordinator. Also before clearance and as deemed necessary, NSTP Coordinator finally inspects apostolate results if they warrant such clearance.
f. NOTE: process your clearance PERSONALLY and BY GROUP: clearance not processed personally means INCOMPLETE and with additional remedial if grade is passing; otherwise, FAILED GRADE IS GIVEN.
14. Photocopy of Specimen of the ID (with picture, signature and address) of any of parents as guardian, authorization of Parents to Official Guardian (if applicable), photocopy of the ID of the authorized guardian (with picture, signature and address) and any proof of COMELEC registration
