    1. R GISTRATIONa. Please complete the attached nrollment Form

    and mail it in with payment in full. Send one n-rollment Form per band (include all participantsand non-participants). The form may be duplicatedfor additional names. All rum ajors from aschool may be included on one nrollment orm as

    well.(Please see item #2 for detailed informationregarding payments)

    b. The On ampus fee includes all housing, meals,and instruction.

    c. The ommuter fee includes instruction only.( eals may be purchased for commuters on a permeal basis at most facilities contact the USA of cefor more information).

    d. The on- articipant fee includes housingand meals.

    e. ue to contractual obligations with host facilities,camp fees will be charged to all attendees regard-less of age or participation. Adults may also auditstudent classes and attend directors workshops.Participants must be at least 10 years of age toattend a Junior High Camp and at least 12 yearsof age to attend a High School Camp.

    f. Once the USA of ce has received your nrollmentorm and payment, an email will be sent to the

    person designated to receive correspondence. Thisemail will include instructions on how to downloadimportant camp information from the online amp

    egistration Tracker.

    2. PA M NT INFORMATIONa. ayment can be made by credit card, school or

    cashiers check, money order, personal check, orschool purchase order. Organizational checks arenot accepted. ake all payments payable to USASummer amps. lease no cashor paymentsmade by individual s uad members (if registeredas a group). A $25.00 fee will be assessed for eachreturned check.

    b. f you are paying with a purchase order, it should bewritten for theentire camp balance and submittedwith a completed nrollment orm. ake sure theschool name appears on the purchase order.DrumMajors submitting purchase orders without a com-pleted nrollment Form will not be registered.

    c. As an added convenience, payment can be madewith aster ard, SA, iscover, or American

    xpress.For payment by credit card, use thespace for credit card authorization on the

    nrollment Form.

    3. CANC LLATION/ADD/CHANG POLICa. Any changes/cancellations after your nrollment

    orm has been processed will need to besubmittedin writingto the USA of ce.

    b. All payments, except a $50.00 per person deposit,are fully refundable if written notice of cancellationis received in the USA of ce at least 14 days prior toyour camp opening. f you do not receivean invoicecon rmation re ecting your revised numbers within48-hours of submission, please follow up with theUSA registration department.Cancellations are of-

    cial only when received in writing and con rmedby the USA of ce.

    c. USA will only accept cancellations in writing.d. Because of facility deposits paid by USA, no-

    shows at camp or cancellations received less than14 days prior to camp will not receive any type ofrefundand will be responsible for the full tuition.

    e. ayments may be applied to substitutes. Thesubstitution must take place at the same time as thecancellation.

    f. Should a no-show individual at a camp have anoutstanding balance (e.g. unpaid school purchaseorder, etc.), full tuition will still be due for the in-dividual even though she/he did not attend camp.

    4. R FUND R qU STSa. A separate refund request for cancellations received in writing at least 14 days prior to the campopening must be submitted in writing to the USAof ce by October 1, 2010. Again, the refund requestis separate from cancellation request. o refund willbe issued before the end of camp or after ecember31, 2010.

    5. ROOM ASSIGNM NTSa. ultiple drum majors from the same school will be

    housed together at camp.b. oom assignments for bands will be made by the

    team representative upon arrival at camp. To expeditethis process, please prepare your own rooming list(two to a room at most locations) and bring it to camp.

    6. CAMP CAPACITa. any USA camps ll to capacity. f bed space is

    not available when you register, you will be noti edimmediately regarding alternate camp dates.

    7. NON PARTICIPANT ADULTSa. irectors, adults, and chaperones should register

    as non-participants on the enrollment form.Therst registered adult from each school will be

    charged at the discounted rate of $125.00. Anyadditional non-participant adults will be billed thenon-participant fee.
