DISPENSING TO INPATIENTS

INPATIENT: who are retained in the hospital so that he/she occupies a bed in hospital for medical care.

DISPENSING: A pharmacy act to remove two or more doses from a bulk drug container and placing them in another (dispensing) container for subsequent use by a patient.

DISTRIBUTION: the supply, delivery, and transitory storage of drug at patient-care areas other than main hospital pharmacy for subsequent patients utilization

Guidelines for distribution: Make it sure that the prescriber's order is original

before starting the dose. Drugs should be dispensed with the compliance of

pharmaceutical technology. Drugs should be stored in place to be accessible

only by pharmacist and nurse.

a. Individual drug order system. Small and private hospital because of unavailability of manpower. Advantages are:

• Direct review of medication order• Better interaction

Disadvantages are: • Delay process• Height cost

b. Floor stock system: Traditional system Separate storage facility in a secured area on each

patient floor. Each area having limited dosage forms such as 10 to 100 Two types:

1) Charge flow stock drugs: charged to the patient account. 2) Non-charge floor stock drugs: coasting in the per day cost

of the hospital room.

Advantages are: Better supervision and control Availability of pharmacist Out-dated drug remove easily Conserve nurse time

c. Complete floor stock system: Cabinet carries both charged and

uncharged . Two system are currently used:1. Floor stock system under supervision of

nurse: 2. Floor stock system under supervision of

pharmacist-satellite pharmacy.

Advantage of the first type are: Ready availability Reduction in nursing time No drugs return to the main pharmacy

Disadvantages of the first type are: Medication errors Drug inventory Drug deterioration More nurse are required

d. Unit dose distribution system: Medications are ordered, packaged, handled, administered,

and charged in multiples of single dose units containing a predetermined amount of drug sufficient for one regular dose.

Advantages are: Safe for patient Efficient and economic for institution. Optimization of professional resources.
