Education Program Supervisors School Administrators/Owners and Officer-In-Charge (Private Pre-Elementary, Elementary, Junior High School) All Others Concerned

1. The Schools Division Office hereby announces the Regional Standards for the ocularinspection and the template for the Affidavit of Attestation to be used during the ocularinspection of the DepEd Monitoring Official.

2. All concerned private schools pre-elementary, elementary and secondary schools shallobserve the said enhanced process in the application of government permit. Kindly seeattached Regional Memorandum No. 138, s. 2017.

3. Immediate dissemination and strict compliance with this Memorandum is earnestlydesired.
