WHAT IS THE HIP FRACTURE PATHWAY?The Hip Fracture Pathway consisting of the Geriatric Fracture Program (GFP)* is a quality improvement initiative focused on improving the care of patients 65+ suffering from a fragility hip fracture. This solution aims to improve patient outcomes, reduce length of stay and improve patient satisfaction (addressing triple aim metrics).
