Important Things to Know

• Plan year is January 1 – December 31, 2014

• To be eligible for benefits, you must be working 35+ hours per week

• Benefits are effective 1st of the month following 30 days

• Eligible dependents include spouse and children (regardless of student status) up to age 26

• Consider your elections carefully. You cannot change your elections until the next open enrollment period unless you have a Qualifying Life Event as defined by the IRS. – Examples include: marriage, divorce, birth, adoption, or change in status

that affects you or your dependents eligibility for benefits.

Medical Plan Design

• BenefitMall offers a single medical plan : Blue Edge HSA – High Deductible Health Plan (HDHP),

– that is qualified for you to have a Health Savings Account (HSA)

• Plan through Blue Cross Blue Shield of Texas (BCBSTX) • Provides one of the largest national networks

• The HDHP is a PPO plan meaning you have the flexibility of going both in and out of network

• HSA offered through HSA Bank• An HSA is an individually-owned, tax-favored savings account that can be used to offset out of

pocket costs including deductible and coinsurance expenses.

Preventive Care

• Preventive care is precautionary and determined by federal legislation

HSA Funding

• You can contribute pre-tax dollars into your HSA account via

payroll deductions.– You can change your HSA contribution on a monthly basis in Exponent (as a Life Event)

• You can make a contribution directly to HSA Bank through a

standard online bank transfer– Look for additional information in your welcome packet.

HSA Bank• Upon enrolling in the BCBSTX HDHP, an HSA account will

automatically be opened in your name through HSA Bank

• You will receive an HSA debit card to use for eligible expenses

• HSA Bank may require you to verify certain personal information such as home address. They will contact you via phone and letter if they need additional information. Common delays include:– The Federal government requires a valid street address to open bank accounts. If you

have a P.O. Box as your address in Exponent, this could cause delays with the opening of your HSA.

– HSA Bank requires a phone number to open an account. Make sure you update Exponent accordingly!

Working Spouse Surcharge

• BenefitMall will add a spouse surcharge of $25 per month if a covered spouse declines medical coverage through his/her own employer and chooses BenefitMall medical coverage.– If a spouse chooses coverage through his/her own employer, or if employer

coverage is not available, the spouse surcharge will not apply.

• Online Acknowledgement in ExponentHR Enrollment System– If you elect spousal coverage for medical, you will prompted to complete an online

acknowledgement attesting whether your spouse has medical insurance available

through his/her own employer. If you elect yes, and enroll spouse in medical, your

rates online will reflect the $25/month spousal surcharge.

– Failure to accurately declare your spouse’s status may result in cancelation of

your medical coverage. Additionally, your claims may be denied and you may

face disciplinary action up to and including termination of employment.
