CP Rochester’s Community Recreaon Program is designed to engage people of all ages and abili- es in fun recreaonal acvies. Our primary goal is to provide opportunies for individuals with a disability to develop recreaon skills and explore interests while providing a break for their caregiv- ers. Although we also offer some group and family-oriented oungs, we encourage parents to take ad- vantage of our respite service. Taking me away from caregiving responsibilies allows caregivers to take care of themselves such as run errands, go to doctor’s appointments, maintain social rela- onships and to re-energize, which ulmately allows for more effecve caregiving. Programs that are in the secon “Programs for Individuals” are intended to be respite in nature and count toward waiver parcipaon. Individuals must parcipate in at least one individual pro- gram each year to remain acve on CP Rochester’s roster. Our programs are community-based and parcipant-driven. We encourage families and parci- pants to make suggesons and provide feedback on the programs that are offered. Please note that the only programs that we offer a staffing rao of 1:1 is for the Individual Swim Lessons, Fun Fitness and Adapve Bouldering. Other programs ulize staff and volunteers for support and over- sight in an age appropriate rao. is: $90 per person

Ages: 3 and up

Instruc+on is provided by our Adapted Physical Educa+on teacher and swim-

ming instructors and is designed for individuals who have special needs. Stu-

dents will receive 30 minutes of instruc+on and will work toward developing

independent swimming skills and increased safety awareness in the water.

Our bowling league is a great recrea+onal and social opportunity. We bowl weekly

and celebrate at the end of the league with a pizza party & award ceremony. No

previous experience is necessary, just an interest in bowling. Individuals will be

grouped into teams and score will be kept each week. Bumpers & bowling ramps

are available so people of all abili+es and experience levels can par+cipate!

Programs for Individuals

Program #4

Gym Play is a social ac+ve environment program su-

pervised by our recrea+on staff here at CP Rochester.

This program will have a variety of sport and move-

ment ac+vi+es available to youth interested in being

ac+ve and exploring new ac+vi+es. This program will

be held the first Friday of each month. Spots are lim-

ited so please register in advance.

Ages: 6 to 16

Dates: May 6th, June 3rd, July 1st

Time: 5:00pm to 6:00pm Ages 6 to 10

and 6:30pm to 7:30pm Ages 11 to 16

Cost: No cost to par+cipant

1:1 Fun, Fitness, and Physical Activity

S P R I N G 2 0 1 6

This fun fitness program provides the opportunity for increased

physical ac+vity levels and skill development by working with a 1:1

instructor. Sessions are geared toward the individual's age and in-

terests with a focus on aerobics, muscle strength, endurance, and

flexibility components. Each session is 30 minutes long and is avail-

able for ages 5-17.

Descrip�on for individuals ages 5-9: Sessions include skill development and opportuni-

+es for increased physical ac+vity through par+cipa+on in ac+vi+es/games. Fitness

levels will be assessed by observa+on.

Descrip�on for individuals ages 10-17: Sessions will involve fitness tes+ng (Brockport

Physical Fitness Test) to assess each individual’s specific needs. ANer assessment, tar-

get areas of fitness will be selected by the individual; games and ac+vi+es will supple-

ment the session.

This unique form of rock climbing goes sideways rather than ver+cal, therefore, no ropes or

harnesses are required. This program is a great way to get youth exposed to the world of rock

climbing in a safe quiet environment. Students will receive 30 minutes of instruc+on and will

work toward developing coordina+on and balance.

(Close toed shoes required)

Visit three floors of hands-on, interac+ve exhibits in science, technology, natural

science, and regional cultural heritage. You can also enjoy the traveling exhibits

of the RMSC during your visit.

Please note, this is a family program and staff is not present at the museum.

2. Complete an Intake Packet for Respite / Recrea+on

3. Submit your completed Intake Packet to the Intake Coordinator

4. Obtain approval for Recrea+on Respite services

In order to par�cipate in Respite Recrea�on Programs, individuals must:

• have Medicaid

• be enrolled in the HCBS waiver

• have received approval for the service from CP Rochester and DDRO

• par+cipate in a Respite Recrea+on Program annually

How to Register for Programs All required intake documenta�on must be received and processed prior to register-

ing for an event or program.

registra�on, staff must ensure that all the required documenta�on is in place for each

person, and at �mes we must follow up with your Medicaid Service Coordinator.

The confirma+on email will include payment details. Sending payment in with your registra+on will not

guarantee a spot in the program. You may call or email to check on the status of your registra+on:

Program Fees We try our best to offer low or no-cost recrea+on programs, however, some ac+vi+es do incur expenses

beyond basic staffing resul+ng in fees which must be paid aNer successful registra+on. This cost applies

to both HCBS/Care at home waiver enrolled AND non-waiver enrolled individuals.

If you are enrolled in the HCBS Waiver or the Care at Home Waiver, you will s�ll need to pay the fee

that is listed for that ac�vity.

If requested, we will provide a receipt for payments made to CP Rochester for Recrea+on Programs. You

may choose to submit your receipt for Family Reimbursement—please contact your Medicaid Service

Coordinator for further informa+on.

Payments or Cancellations Payments or cancella+ons must be made by the program’s registra+on deadline. If we have not received

payment one week prior to the event’s date or start of the session and you have not contacted us, we

will assume that you are not going to par+cipate in this program anymore. Past due payments will affect

registra+on for future recrea+on programs. In the event that a program is cancelled, we will issue a

voucher that can be used toward future recrea+on programs; we do not issue refunds.

Project Access Transportation Project Access is a grant supplemented service that provides, low cost transporta+on to individ-

uals with disabili+es living with family in Monroe county.

We would like to introduce you to Ray’s Social Club; an inclusive social outlet for young adults ages 17-21. Ray’s Social Club brings together teens and young adults twice per month for discussions, social activities, friendship and sup-port. Social activities throughout the community are planned in advance with as-sistance from CP Rochester staff. The social activity takes place on the 1st Satur-day of each month. Additionally, members of the Club take part in monthly facilitat-ed group discussions. Guided by staff with a counseling background, members are able to talk about challenges, concerns and frustrations that they are experiencing and gain peer support. The group discussions take place on the 3rd Thursday of each month. Regular attendance is a requirement of Ray’s Social Club. We are ex-cited to share this opportunity with you!
