Purpose of the CCIP & CCIP Standards

• CCIP was designed to support Advanced Networks and FQHCs in the development and implementation of network-wide capabilities to improve primary care

• CCIP was envisioned to complement PCMH+ and other shared savings programs by focusing on capabilities that could lead to improvements in key shared savings program measures

• A full description of CCIP can be found in the Report of the Practice Transformation Task Force, a multi-stakeholder SIM Advisory Body

CCIP Wave 1: Lessons Learned

1. Highly targeted technical assistance is needed to meet the requirements of the CCIP Standards

2. Each Participating Entity (PE) has unique strengths and needs that require specific approaches to technical assistance

3. Peer Learning is an important component of success in CCIP

4. Online Learning Materials are important to support knowledge distribution across Participating Entities

Purpose of the CCIP Transformation Awards

CCIP Transformation Awards are intended to provide targeted, direct funding to PCMH+ PEs so that they can make investments to achieve the CCIP standards across their networks. Funding should be in support of patient care for all populations that are attributed to the PE, regardless of payer.

CCIP Wave 2 Transformation Awards: Eligibility

Eligibility to apply is limited to those entities that have met the eligibility requirements to participate in Wave 2 of PCMH+, Track 1. Entities are not eligible to apply if they are participating in the CCMI funded Practice Transformation Network.

CCIP Wave 2 Transformation Award Details

•Awards are available up to $750,000

•Awards will be considered a sub-award of the SIM grant

•Quarterly Reporting will be required to communicate progress against project plan

• Invoices will be submitted monthly to SIM office within the Office of Health Strategy

How can the Transformation Awards be used?

•Technical Assistance

Transformation Award Priority Funding Areas

Targeted technical assistance focused on the development of a particular capability

Technical assistance pertaining to any aspect of the Core or Elective Standards to support change management and process re-engineering

Community Health Worker Integration

Contracted or employed staff to address a core or elective standard, for example Community Health Workers or pharmacists

Shared community or care delivery resource solutions undertaken in partnership with other awardees

Customization and deployment of social determinants of health (SDOH) data collection tools

Care management and analytic software tools

Subscription to alert services such as Patient Ping
