Stay on track with Aetna Navigator® First things first. Your member website is hands down the best place for handling benefits business. From claims information to finding a specialist, the Aetna Navigator website is your online health hub. You’ll find information and tools to help you make the most of your plan benefits. It’s also where you’ll find the resources to help you save money on health care.

Follow these simple steps to register. Then, each time you access the site, your information will be personalized for you.

1. Have your Aetna member ID card handy

2. Go to aetna.com and click “Login.”

3. Follow the prompts to create your user name and password.

Online Resources

Download the Aetna Mobile appAccess the most popular features of Aetna Navigator on your Android™ or Apple® device, and have a digital copy of your member ID card with you at all times with Aetna Mobile. You’ll use the same log-in information you use for the member website.

Text Apps to 23862 (data and messaging rates may apply) to download Aetna Mobile.

On Navigator, you’ll be able to:

Find network doctors Remember, your share of the cost is lower when you use network doctors. Finding them is quick and easy with our DocFind® search tool, which is prefilled with all your plan information. Log in to aetna.com > Find Care. Then, choose the type of doctor or facility you are looking for.

Locate walk-in clinics and urgent care centers close to you These facilities can handle non-life-threatening issues when you can’t see your regular doctor. And, they are much less expensive (and usually faster) than a trip to the emergency room. Log in to aetna.com > Find Care.

View your claims and Explanation of Benefits (EOB) statements Log in to aetna.com > Manage Claims.

View or replace your Aetna ID card You can view your card online by logging in to aetna.com > ID Card. If you need a replacement ID card, you can request one by calling Aetna at the number below:

Live healthier with the Omada® Diabetes Program Since the launch of our virtual Diabetes Prevention Program, more than 3,500 Dow people have taken action to reduce their risk for diabetes by participating in this innovative online healthy living program!

Omada is based on our Diabetes Prevention Program clinical trial, which showed that people at high risk can delay or avoid developing type 2 diabetes by losing weight through diet and exercise. Participants who made these healthy lifestyle changes reduced their risk of developing diabetes by 58%! Plus, they lost 5% of their body weight — and maintained that loss for a year or more!

If your doctor has said you’re at risk for type 2 diabetes or heart disease, and you participate in a Dow medical plan, we will cover the entire cost of the program for you and your eligible adult dependents — a $700 value.

See if you’re eligible to participate You can contact the HR Service Center at 1-877-623-8079 with questions.

1. You must be covered under a Dow medical plan.

2. You must complete the one-minute risk screening questionnaire, which can be accessed at www.omadahealth.com/dow.

3. If you qualify to participate based on #1 and #2 above, Dow will cover the entire cost of the program.

You may not qualify to participate if you’re not covered under a Dow medical plan, or your body mass index (BMI) is less than 24.

Important update for Medicare-eligible retirees: CMS/Medicare will cover diabetes prevention programs for Medicare participants.

Your participation in Omada is voluntary, and your responses will be handled confidentially. No individually identifiable data will be shared with Dow. Your responses to the screening will be stored by Omada during the term of the program.

Terminal Illness and End-of-Life ServicesThe Conversation Project about end-of-life wishesThe Conversation Project is a campaign with a goal that is both simple and transformative: to have every person’s end-of-life wishes expressed and respected. Too many people die in a manner not of their choosing. The Conversation Project offers people the tools, guidance and resources they need to begin talking about their wishes and preferences.

Dow Health Services is working in partnership with The Conversation Project to bring this important resource to you and your family. The campaign does not promote any specific preference for end-of-life care; instead, it seeks to encourage and support people in expressing their end-of-life wishes for care.

Co-founded by Pulitzer Prize winner Ellen Goodman, in collaboration with the Institute for Healthcare Improvement (IHI), The Conversation Project’s cornerstone is a website that offers visitors a Conversation Starter Kit and asks people to submit their personal stories to be shared at www.theconversationproject.org and via YouTube®, Twitter® and Facebook®. By sparking

cultural change at the kitchen table — not in the intensive care unit — The Conversation Project hopes it will become easier for people to communicate end-of-life wishes that can be expressed in advance and respected at the end.

While the subject is end of life, this conversation is best carried out as early as possible. This way, people can discuss their wishes without pressure, when they are still relatively young and healthy.

“We are all mortal, yet as a society, we have been painfully slow in recognizing how many of the people we love are not dying in the way they would choose: in comfort, among people who care about them, and engaged in what matters most for as long as possible,” said Goodman. “We believe this is the time to change the cultural norm from not talking about how we want to live at the end of our lives to talking about it.”
