Registration checklistPlease bring the following items with you to registration. 4 Child’s birth certificate 4 Child’s social security card 4 Child’s immunization records (See below) 4 Parent/guardian identification 4 Proof of residency documents (2)* 4 Proof of custody (if parents are divorced/ separated or if there is a custody issue of any kind) 4 IEP and ETR (if applicable)
* Proof of residency documents include any “official” mail coming to the parent /guardian’s address that includes their name, address and a current date. “Junk” mail will not be accepted. Acceptable documents may include, but are not limited to: bank statements, credit card statements, court documents, leases, local/state/federal documents, pay stubs, personal bills, police reports, postal change of address, sales agreements, utility bills and voter registration cards. All residency documents must not be older than 60 days.
Required immunizationsThese may be scheduled with your family physician. Akron Health Department offers a walk-in clinic: scphoh.org/immunizations.
