INTERIM REPORT ON METRO BLUE LINE ACCIDENTS
This report serves as a preliminary progress update on Metro staff efforts to evaluate accidents on the Metro Blue Line and actions taken to date to identify additional safety measures to mitigate the accidents.
BACKGROUND
In accordance with a motion passed by the Metro Board of Directors at the August 6, 2012 meeting, staff invited and convened affected stakeholders, including Los Angeles Department of Transportation, Los Angeles County Public Works, The City of Long Beach- Traffic Engineering, Union Pacific Railroad (UP), Los Angeles Sheriff's Department (LASD), City of Compton, and California Public Utilities Commission (CPUC}, to establish a task force to assess the Metro Blue Line (MBL), and to recommend additional safety enhancements. The extent of safety enhancements identified by the task force will be based on data contained in the MBL quarterly accident report which is distributed the agency stakeholders list above. The latest quarterly report is included in Attachment A.
The Metro Blue line is comprised of essentially two alignment types - midcorridor and street running . These two alignment types are prevalent across North American light rail transit systems. Mid-corridor alignment, describes areas where the train crosses intersections that have gates, ra ilroad flashing lights, and bells installed. Trains operate at speeds up to 55 mph in the mid-corridor segment of the line. Street running alignment describes areas where trains operate at a maximum speed of 35 mph. Instead of railroad gates, flash ing lights, and bells, street running alignments have traffic signals and special train signals (known as "bar signals") installed to ensure a safe interface between vehicles, pedestrians and trains. The mid-corridor section is between the
Washington Station and the Willow Street Station. The street running sections are between the 7th/Metro Center Station and the Washington Station in Los Angeles, and as well as south of the Willow Street Station in Long Beach. See Attachment B for a map of the MBL and the locations of the alignment types.

The decision to classify an intersection as street running or mid-corridor is made during the preliminary engineering design phase in close coordination and agreement between the local traffic agency, CPUC, and Metro. The decision takes into account factors such as traffic volumes, train speeds, available rightof-way, congestion at adjacent intersections, etc. Ultimately, approval of all intersection classifications rests with the CPUC and in the case of the MBL every intersection was approved by the CPUC.
Over the last 1 0 years, numerous safety enhancements have been implemented to mitigate accidents on the MBL; the specific improvements are described below. The implementation of these measures resulted in a 62°k reduction of train/vehicle accidents in the mid-corridor portion of the line and a 49% reduction of train/vehicle accidents in the street-running portion when comparing the last eleven years to the first eleven years of MBL operation. Metro's demonstration project to install four-quadrant gates in 1998 at the 124th Street grade crossing was effective in reducing vehicle accidents on the MBL, and acted as the catalyst for the CPUC's adoption of this technology in their regulations. The safety devices installed at the MBL grade crossings exceed the minimum requirements of the CPUC.
Metro has been challenged with reducing the train and pedestrian accidents in both alignment types. The rate of suicides has increased from 8 incidents in the first eleven years of operation, 1990-2001, to 22 incidents in the last eleven years of operation, 2002-2012. This represents a 175% increase in the number of suicide related incidents.
The quarterly accident report presents the train versus vehicle and train versus pedestrian trend over the latest rolling 10-year period. The data in the quarterly report ending June 30, 2012, indicates an increasing trend of accidents with pedestrians, with most ending in fatalities. A statistical analysis of the fatalities and suicides is shown below.

ACTIONS TAKEN
Since the opening of the MBL, numerous enhancements have been implemented to mitigate accidents. Photographs of many of these enhancements are included in Attachment C.
Between 1990 and 1995, the following measures were implemented:
• Installed a "Cyclops" light on all trains to enhance the visibility of approaching trains for pedestrians and motorists

• Reduced the height of the right-of-way fencing at highway rail grade crossings to improve visibility for train operators of the grade crossings
• Changed the mechanical horn on the trains to an electronic horn to provide a more focused warning
• Installed additional flashing lights and bells at grade crossings to provide added active visual warnings on all approaches to the crossing

• Installed active "No Left Turn" signs in street-running alignments to reinforce awareness of the prohibited movement
• Modified the traffic signal system in Los Angeles to reduce the possibility of conflict between motorists turning left across the tracks and trains
• Installed "Swing Gates" at several high-traffic pedestrian crossings, forcing pedestrians to stop prior to entering the crossing
• Modified the headlights on all light rail trains to flash alternately when the horn is sounded to increase the awareness of the train's presence
• Installed median islands at several intersections to prevent motorists from going around lowered railroad gates
Between 1996 to 2000, the following measures were implemented:
• Installed "Four Quadrant Gates" and Vehicle Detection Loops at 124th Street
• Modified the design of train "T" signals in the City of Los Angeles and the City of Long Beach to "Bar" signals to reduce any possible confusion for motorists
• Installed automatic photo enforcement cameras at 17 intersections in mid-corridor to issue citations to motorists who ignore the warning devices
• Replaced passive "No Left Turn" signs with active LED "Train Coming" Signs in the City of Los Angeles
Between 2001 to 2004, the following measures were implemented:
• Expanded the installation of active LED "Train Coming" Signs in the City of Long Beach
• Installed active "Look Both Ways" sign at Vernon Station to warn pedestrians
• Expanded the photo enforcement program to "street running" intersections in the City of Los Angeles

• Installed pedestrian gates and swing gates at additional locations (Vernon, Willow)
• Designed and installed a 1.2 mile center fence in the City of Long Beach to prohibit pedestrians from crossing the tracks between intersections
In 2006, at the request of the City of Los Angeles, an Ad Hoc Committee, comprised of representatives from the City of Los Angeles, City of Long Beach, Metro and LASD, was established to assess the need for additional safety measures. There were 22 measures evaluated by that Committee (see Attachment D for complete list of recommendations). The following 12 of the 22 were deemed to be feasible for implementation at the time and were completed:
• Restripe faded pavement markings at all intersections • Install colored stamped crosswalks • Evaluate current Programmable View (PV) heads on all through signal
heads • Evaluate alternate train speeds • Review the 'defensive driving' portion of operator training to determine if
changes are warranted to emphasize these skills • Conduct on a regular interval targeted enforcement of pedestrian and
vehicular traffic laws at intersections on a rotating basis (On-going implementation)
• Change left turn phasing from lead to lag to be consistent at all times • Use of train "horn" in addition to the gong • Expand photo enforcement to all street-running intersections • Enhance public education • Install in-ground pavement flashers • Produce public service announcements for left turns
The local jurisdictions did not deem the following 8 to be feasible:
• All way red when the train approaches • Eliminate left turns at selected intersections • Eliminate left turns altogether on Washington Boulevard, Long Beach
Boulevard, Pacific Avenue. • Install an active LED "No left turn" graphic sign on a pole in between our
tracks at driver's eye level height that would flash every time the arrow is red and turn off when the arrow is green
• Evaluate crossing gates • Increase duration of left turn phase • Install far-side mirrors for motorists in left-turn lanes • Grade separate the street-running alignments

The following two were not implemented, but are being re-evaluated by the current Metro Blue Line task force described below.
• Replace the green traffic indication with "up" arrows • Re-introduce billboards depicting left turn safety
Between 2007 and 2012, the following measures were implemented:
• Installed four-quadrant gates at the five intersections: Elm Street, Greenleaf Boulevard, Compton Boulevard, Alondra Boulevard, and Myrrh Street that cross the MBL in the City of Compton
• Installed active "TRAIN coming" signs for cross traffic at all the intersections that cross the MBL tracks in City of Los Angeles and Long Beach
• Closed three grade crossings on Flower Street completely eliminating the possibility of accidents at these locations
• LASD increased monitoring and enforcement along Washington Boulevard to address illegal left turns by motorists
• Safety ambassadors placed at Grand, Vernon, Florence, 1 03rd, Imperial, and Artesia grade crossings
• Flyers and door hangers distributed to residents along the MBL
• Modified the operating speed at Vernon, Florence, 1 03rd Street, Wilmington, Wardlow and Artesia stations to provide additional reaction time for operators when they observe unsafe behaviors by pedestrians. Speed signs have also been posted to remind operators of modified operating speed.
Throughout the history of MBL operations, we have maintained a continuous enforcement and public education outreach program. The outreach program included rail safety videos, public service announcements, and safety presentations in the communities and schools surrounding the MBL. The enforcement program included focused enforcement of grade crossing traffic laws, trooper on the train, and motorcycle and bicycle patrols to cite motorists and pedestrians who do not obey the grade crossing warning devices.
NEXT STEPS
This fiscal year, we have budgeted $725,000 to start the implementation of active "TRAIN" signs at pedestrian crosswalks which cross the tracks at all stations between 7th/Metro Center and Washington, as well as south of Willow Street. Additionally, Metro plans to design and install pedestrian gates and swing

gates in the mid-corridor portion of the MBL. In addition to the above safety enhancements that have already been completed and are in progress, staff is chairing a task force of safety experts from the cities along the alignment, the County, CPUC, Union Pacific Railroad, the Los Angeles Sheriffs Department (LASD), and Metro staff to further evaluate safety enhancements. This task force will also evaluate measures to mitigate suicide related incidents by discussing strategies for signage with local suicide prevention organizations and training with LASD staff. The task force's recommendations will be presented to the Board at the November 2012 meeting. A combination of engineering, enforcement, and education efforts has had a positive impact on the safety of the Blue Line. Staff will continue to review emerging technologies as they become available to reduce accidents on our rail lines.

In Fiscal Year 2010. this report was revised to include Metro Blue Line accident data for the latest 1 0-year period during which Metro has implemented several safety enhancements. SUMMARY OF METRO BLUE LINE TRAIN I VEHICLE AND TRAIN I PEDESTRIAN ACCIDENTS
Corporate Safety is responsible for the collection, maintenance, and distribution of the accident/incident data. This report, Summary of Metro Blue Line TrainNehicle and Train/Pedestrian Accidents is part of the trending performed by LACMTA. With this report, Corporate Safety monitors and analyzes the trends and patterns. In the past, trending has resulted in implementation of grade crossing safety improvements such as the fiber optic trains signs along Flower Street and Washington Blvd, the four quad gate demonstration project, photo enforcement program, new legislation, and public education programs. Corporate Safety will continue to make recommendations and improvements to the rail system as necessary.
Upon assuming the responsibility of generating the report, Corporate Safety reviewed accident data dating back to Jan 1997 and made corrections in the database which were reflected in the first quarterly report prepared by Corporate Safety (quarter ending June 2003). It was not possible to confirm the accident data contained in the database dating prior to Jan 1997 because the source of the originating data was not thorough or complete.
The following contributing factors codes are used in the report:
L T Vehicle entered trackway from left turn lane. RT Vehicle entered trackway from right tum lane. UT Vehicle attempted to make a U tum on a street perpendicular to the trackway. RS Vehicle ran through a red traffic signal or stop sign. FLB Pedestrian violated flashing lights/bells. AE Encroachment by vehicle into the trackway, other than by turning onto the tracks in front
of a train or by running through a red traffic signal or stop sign. RG Vehicle or pedestrian ran around a down gate or broke through crossing gate. TR Pedestrian trespassing on the right-of-way. HR Vehicle left accident scene without stopping DR Intoxicated driver or pedestrian. ST Two or more trains passing through the crossing. SU Suicide. PD Police Department vehicle involved in accident. FD Fire Department vehicle involved in accident. SO Vehicle or pedestrian traveling in same direction as train. EB Vehicle or pedestrian entered trackway in eastbound direction. WB Vehicle or pedestrian entered trackway in westbound direction. NB Vehicle or pedestrian entered trackway in northbound direction. SB Vehicle or pedestrian entered trackway in southbound direction.
The direction of travel of the MBL train is either northbound (track 1) or southbound (track 2). In the "Contributing Factor(s)" column, the geographical direction of travel of the vehicle or pedestrian is used.
There are two types of accidents, Train vs. Auto (TA) or Train vs. Pedestrian (TP). Incidents involving bicyclists are coded as TP; incidents involving motorcycles are coded as T A. Incidents involving objects are not included in this report. Incidents, which only involve mirror damage to either the Train or the vehicle, are noted in a separate table in the back of the report. Same for incidents categorized as possible pedestrian incidents. These incidents result in no pedestrian found at the scene when either the Operator or SupeNisor investigates but no conclusion can be made as to whether an incident occurred or not.
Accidents with an asterisk (*) to the right of TA or TP are either new in this quarter or updates/corrections to previous reports.
