Assessing Female Genitals
Infants
• Clitoris may be edematous and enlarge• May be a white vaginal discharge
Children• Parent approval• Adolescent – limited for inspection• Clitoris - common site for chancres• Stages of pubic hair development
during puberty
Elderly• Clitoris - common site for CA lesions• Vulva - atrophied, easily irritated• Ovulation and estrogen production
ceases• Prolapsed uterus• Uncomfortable for examination position

Common type of vaginal infections• Monilial or yeast infections – thick,
white, curdy patchy discharge• Trichomonal infections – profuse,
watery, gray or green, odorous discharge.
• Bacterial infection – odorous discharges

Infants• Foreskin (uncircumcised) – normally
tight
Children• Parent approval• The cremasteric reflex cause the testes
to ascend into the inguinal canal.• 5 stages of development
Elderly• Penis decreases in size / firmness.• Testosterone is produced in smaller
amounts• More time to achieve erection• Seminal fluid is reduced• Problems with beginning and ending
the urinary stream are usually result of prostatic enlargement.
Client Preparation
• Empty the bladder• Emotionally prepared• Explain the procedure• Eye contact with the client• Proper position• Companionship• Draping• Warm hands• Be gentle

Assessment of Anus, Rectum & Prostate
Inspection:• In side lying position, inspect the sacro-
coccygeal and perineal areas
Smooth, intact, free from feces. Anal mucosa is deeply pigmented; coars moist and hairless.
Palpation:• Lubricate gloved index finger of
dominant hand. As the sphincter relaxes, insert finger into the anus. Insert further and palpate as much rectal wall as possible in sequence. Palpate the surface of prostate region. Note the nodules, tenderness, size and shape.
