Overview

A Message from the Health Commissioner ... In May of 2010, we opened the Fort Wayne-Allen County Department of Health Medical Annex. This facility houses all of the communicable disease-related clinical and investigative services provided by the department. The Allen County Commissioners provided the building and the Allen County Council provided the funding for renovations and remodeling to create this state of the art public health facility. Our goal is to become a Center of Excellence for Clinical Public Health. We anticipate providing the following services at the Medical Annex:

Patient Assessment, Treatment and Education Track: We will continue to provide clinical services for sexually transmitted diseases (STD), active and latent tuberculosis, screening of primary refugees and childhood immunizations. We will be adding a multi-disciplinary lead poisoning clinic, adult immunizations, travel immunizations and education, well child/adult visits for new refugees (paid through federal funds), STD education, and reportable communicable disease evaluation and treatment for the un-resourced who are excluded from work.

Community Health Data Collection and Research Track: We will continue to investigate all reportable communicable diseases. We are in the process of implementing a new infections disease database (INEDDS) that will allow us to perform more timely reports regarding both local infectious diseases and risk factors as well as resource utilization and clinical demographics.

Onsite Research and Clinical Internships: We will continue to provide clinical internships for nursing and medical assistant students. We will be providing clinical clerkships for Indiana School of Medicine fourth-year students and for other students who would like clinical experience or would like to do a research project. Providing such clinical and investigative services will allow our department to more quickly and effectively identify and contain any contagious disease outbreaks, identify new trends in communicable diseases and prepare the next generation of community health and epidemiologic specialists.

Overview

A Message from the Administrator ... I can only sum up the year 2010 with the description of “significant accomplishment”! Every year seems to bring its share of challenges, emergencies, new projects and change – and this year was no different. Upon the heels of concluding the projects associated with the 2009 H1N1 pandemic, we became heavily involved in finalizing the plans and moving details for our new combined clinical location – now known as the Medical Annex. In early May, we transitioned all clinical divisions to the new building on New Haven Avenue and have been working on a streamlined work environment ever since. This was a great move for us in the context of a strained economic climate and we are extremely grateful for all who supported this effort. We are now heavily engaged in working toward the next “big move” – which will be the planned move of many of the departments currently located in the City-County Building to the 200 East Berry Street location to occur in 2011. All remaining divisions of our department currently in the City-County Building will move to the East Berry Street location. The year also included many highlights – not the least of which included several large computer software and database updates, many collaborative working relationships and projects with partner agencies, many staffing shifts and changes, and continued work toward better enforcement powers in our environmental divisions to ensure consistency and the overall safety of our community. Further, changes in state statutes affecting multiple divisions took effect this year as well as changes in local ordinances tasking us with new endeavors. The most pleasing and gratifying thing about working at this Department, though, is how well the staff work together to accomplish every single goal (whether small or large; whether liked or disliked) in a collaborative manner. The staff we have is incredible and as I have said many times – I am very proud to work with all of them. What a group of caring, sharing, and positive folks! Keep up the great work! In conclusion, I must say that public health has not been in such a tenuous situation with regard to depleting funding streams in the context of increasing required duties and tasks for quite some time. It is incumbent upon all of us in the public health profession to work together to use our resources efficiently in order to do our best to protect the community. We must seek out ways to protect the profession of public health as it is the basis of a healthy environment in which to live, work, and enjoy. So, that said – what awaits us all for 2011? I look forward to it..

2010 will be remembered for many things, but none more than the opening of the Fort Wayne-Allen County Department of Health’s Medical Annex. Combining all the department’s medical clinics into one site, the Annex allowed for greater efficiencies and flexibility for the department’s staff and resources. Patients also enjoyed more privacy, free parking and access along the Citilink bus route. The nearly 15,000-square foot building, a former credit union, underwent $1.8 million in renovations that included all new ventilation, lighting, electrical, an elevator and plumbing on two floors. A special grand opening event was held on June 21. While the clinical divisions were referred to separately throughout 2010, a name change was implemented at the end of the year to better reflect their breadth of services. The Clinical Services Division now includes both the Sexually Transmitted Disease and Infectious Disease clinics; Community Health Nursing became Immunization Services; and the Case Management and Field Investigative Services Division became the Community Health and Case Management Division. Starting with a radon awareness event in January and ending with World AIDS Day activities in December, 2010 was also a year of many public health initiatives. The department handed out more than 500 free radon test kits in 2010. Results showed that 50 percent of homes tested have elevated levels of radon. In February, the Pollution Control division hosted its annual workshop for contractors involved in on-site sewage disposal system design, inspection and installation. In March, the department partnered with local hospitals and community agencies to organize Rx Hope, a free disaster care symposium for volunteers and medical aid workers headed to Haiti. The event was held at the Public Safety Academy of Northeast Indiana and participants got an overview of general safety issues and infectious disease risks, communication in a disaster environment, and psychological first aid. More than 80 people also took advantage of free vaccines recommended for

travelling to the earthquake-ravaged country. March also saw the release of the County Health Rankings report by the University of Wisconsin and the Robert Wood Johnson Foundation. Allen County ranked 33 out of 92 counties in Indiana for health. While there was much to celebrate, the report also highlighted some of the physical, social and environmental issues at the root of many public health problems. The environmental health divisions used those findings to begin focusing on the Built Environment as part of a healthy community. 2010 saw the expansion of the Healthy Homes program to include fall risk assessments for the elderly. This comprehensive program also partnered with a number of agencies on a grant-funded study of developmental delays in lead poisoned Burmese children. Results of the study were expected in 2011. The Food and Consumer Protection Division presented workshops to food establishment personnel and the public on proper hand-washing and glove use as well as farmers’ market and home-based vendor regulations. During the month of June, the department also had a special HIV testing day at the Medical Annex. The event brought in 25 for testing services. About this time the department also began offering childhood lead screenings on Tuesdays at the Medical Annex. June also marked the official start of mosquito season and a special weeklong awareness campaign to bring attention to the dangers of mosquito-borne disease. The Vector Control and Environmental Services Division conducted its seasonal mosquito control program by treating bodies of water with larvacide and trapping and testing adult mosquitoes for West Nile virus. Spraying for adult mosquitoes, also known as adulticiding, was performed 12 times in 2010 as a result of mosquito samples testing positive for the disease. The first of Allen County’s two human cases was reported in September. September also saw the department participate in the third annual Fort-4-Fitness event, the inaugural Public Safety Day at the Public Safety Academy and its own “Fall Brawl” social event for staff and their families. It was also the start of the flu vaccine season and the department once again urged children and adults to get vaccinated. Beginning in November, the department partnered with Parkview Health and the local school systems to provide Flu Mist to school children. More than 7,700 doses of the nasal

spray vaccine were given as a result of the effort. On the same day the department was promoting the Flu Mist campaign at a media event at Meadowbrook Elementary School, another public health initiative was underway. November 18 was the Great American Smokeout and the department took advantage of the opportunity to encourage smokers to kick the habit. Information on tobacco cessation classes and support programs was offered and a rally in support of a state-wide smoking ban was held by Tobacco Free Allen County at the Allen County Public Library. During 2010, the Emergency Preparedness Division continued to work closely with first responder agencies and other community partners to strategically prepare against disasters. The Vital Records Division also conducted trainings with funeral directors and local physicians to prepare for the transition to an Electronic Death Record in 2011. In 2010, the Communication Division responded to more than 240 requests from the media and issued 35 news releases resulting in dozens of stories. The department also continued to educate residents on important public health topics through the weekly radio program “Health 360” on Northeast Indiana Public Radio. Numerous presentations about the department’s programs and services were made to key stakeholders and community groups. The department started sending text and email alerts to residents through a service called Nixle. New features

were added to the department’s Web site. Video conferencing equipment was installed in all three buildings. Staff members received training in computer software such as Excel, National Incident Management System (NIMS) compliance and electronic mail retention. The department also continued its partnerships with local colleges and universities, enabling medical students, nursing students and other interns to gain practical on-site work experience. 2010 also saw its share of new policy initiatives, programmatic changes and staffing changes. The department partnered with Park Center to provide mental health services to at-risk clients in its clinics, but discontinued the program due to lack of client interest and a high no-show rate. Faced with shrinking property tax revenue and a loss in grant funds, the department looked to offset those deficits in 2010 by billing Medicaid for some of its immunization and childhood lead poisoning services. At the end of 2010, the department was preparing to implement a new clinical fee schedule and organize a move of its administrative offices and environmental health programs from the City-County Building to the Renaissance Building on East Berry Street.

Vaccines for TeensFresh on the heels of the H1N1 vaccine effort, the department found itself in another vaccine campaign in early 2010. This version of “March Madness” involved a month-long special vaccine clinic to help get teenagers caught up with newly required vaccinations. Beginning with the 2010-2011 school year, students in grades 6 through 12 were required to be vaccinated against varicella (chicken pox), Neisseria Meningitis (Meningococcal disease) and pertussis (whooping cough). To help families meet the requirement, the department partnered with the Allen County school systems, local healthcare providers and Super Shot Inc. to provide all three vaccinations free of charge. The department used TV ads, posters, and letters to parents to help get the word about the clinic and the new requirements. During the month of March, 4,000 people were vaccinated at the Carew Medical Park site.

Clinical Services

Highlights in 2010• Merged clinical operations of the Infectious Disease Center and the STD Clinic into one clinic setting at the Medical Annex

in May of 2010. While the divisions were referred to separately throughout 2010, they were managed as one and plans were made to implement a name change to reflect the true merge into one collaborative and cohesive department in 2011.

• Prepared to implement fee for service charges in early 2011.• Partnered with local and regional universities to provide clinical sites to nursing/medical students from Indiana University

Purdue University-Fort Wayne, University of Saint Francis, Ball State University, Indiana Wesleyan University, University of Southern Indiana and Huntington University.

In support of the department’s overall mission and for the benefit of the community, the Clinical Services Division aspires to be a recognized leader in the intervention of the spread of infectious disease and to reduce the complications of these diseases, as well as

provide timely and comprehensive initial health services to the refugee population. Clinical services are provided in a professional, confidential and holistic manner and include prevention, screening, treatment and education.

education, referrals and follow-up.• Continue to identify, treat, and cure patients with active pulmonary tuberculosis.• Provide timely and thorough case investigation and reporting of mandatory reportable communicable diseases.• Assess communicable disease occurrences for local trends and implement strategies to decrease disease transmission• Assist with department-wide plans for emergency preparedness as it relates to communicable disease and mass contact

investigations.

In support of the department’s overall mission, The Community Health & Case Management Services Division strives to be a leader in the

community through the treatment, education, and prevention of reportable infectious disesases in Allen County, while treating each client in a holistic, unique and professional manner.

Emergency Preparedness

Highlights in 2010 • Participated in the School Vaccine program, with approximately 6,200 doses given from November through early 2011. • Continued to purchase preparedness supplies and equipment with available grants for field operations.• Achieved a score of 95 on the Indiana State Department of Health (ISDH) Local Technical Assistance Review for 2010.• Met the ISDH requirements for the 2010 grant period, which included preparedness training, drills, and exercises.• Served as a member of the Allen County Community Organizations Active in Disaster.• Served as a member of the Allen County School Safety Committee.• Continued our local, state, and federal Medical Response Corp partnerships.• Maintained our membership numbers in the Allen County Medical Response Corp.

Highlights in 2010• Conducted monthly in-house educational training sessions for staff training and development. Staff continues to refine their

skills in educating and enforcing food safety as well as preventing disease transmission during tattoo and body piercing procedures.

• Presented ServeSafe Food Protection Manager Certification Course to approximately 80 key food protection personnel from area food establishments.

• Presented open workshops to food establishment personnel and the public on Proper Handwashing and Glove Use as well as Farmers’ Market and Home Based Vendors Regulations.

• Conducted training with approximately 130 Indiana University Purdue University-Fort Wayne Hospitality Management students regarding food regulation issues.

• Conducted training with 50 nursing students concerning foodborne illness surveillance.• Participated in Fort-4-Fitness event by displaying safe produce handling techniques. • Sent four staff members to Epi-Ready Training in Foodborne Illness Response Strategies in connection with Indiana State

Department of Health (ISDH) Epidemiology, ISDH Laboratory, and ISDH Food Protection Program Division.• Developed a plan in conjunction with other agencies to respond to food safety security issues.• Worked to identify food deserts and food insecurity areas in the community.

Healthy Homes

Highlights in 2010• Partnered with the Indiana University School of Medicine, the Fort Wayne Medical Education Program, Parkview Outpatient

Pediatric Rehab Center, East Allen County Schools and the St. Joseph Community Health Foundation to conduct a three-pronged research study focusing on the effectiveness of early academic intervention, speech intervention and a traditional developmental assessment tool for Burmese refugee children who have been impacted by lead exposure.

• Assisted the City of Fort Wayne with the successful implementation of the HUD Lead Hazard Control grant.• Began working on a plan to expand healthy homes issues to include injury prevention in the elderly population, principles of

the built environment and radon outreach and education.

Goals for 2011• Reduce lead hazards in homes in Allen County through timely lead case management services, lead risk assessments and lead

hazard remediation.• Increase the number of healthy homes in Allen County through the increased awareness of the Allen County Healthy Homes

Program by both our clients and the community as a whole and through remediation of health problems found in the home.• Provide injury prevention services to the elderly population who are aging in place.• Increase the Medicaid blood lead screening rate in Allen County.• Increase the identification of asthma cases and triggers in homes in Allen County through screening and referrals.• Identify non-traditional sources of lead poisoning in the refugee community• Increase staff knowledge of lead, healthy homes and built environment issues.

The Allen County Healthy Homes Program is a collaboration between the Department of Health and St. Joseph Community Health Foundation.

In support of the department’s overall mission, the Healthy Homes Program strives to be a leader in establishing healthy home

environments through compassionate, professional and timely education, identification, and remediation of home health hazards and the case management, mitigation, surveillance and prevention of the associated health effects for the residents of Allen County.

HIV/STD Prevention

Highlights in 2010• Reported 3,086 STDs in 2010 data surveillance by District III (gonorrhea, chlamydia, syphilis and HIV). STDs represent the

largest percentile of all reportable communicable diseases. • Implemented utilization of the state’s new Internet-based Statewide Information Management Surveillance System (SWIMSS)

for the HIV/STD Prevention Program in February 2010.• Completed move to Medical Annex Building from the City-County Building in May 2010. This move united all the Department

of Health medical clinics under one roof.• Collaborated with community partners to promote awareness and prevention of HIV/STD at area events including: AIDS Walk,

National Day of HIV Testing, Homeless Health Fair, World AIDS Day, and various Health 360 radio segments.• Collaborated with all Medical Annex divisions on the fee for service project. (The intent of this project is to help offset funding

reductions and has a targeted implementation date of early 2011.)• Began receiving electronic lab reporting, ELR, in SWIMSS the later part of the year. Some public and private laboratories

commenced utilizing INEDSS to upload lab reports required to be reported per Indiana Code 410.• Provided HIV testing to 947 high-risk clients, following the stricter testing criteria implemented in 2009 due to reduced HIV

grant funding.

In support of the department’s overall mission, and for the benefit of the community, the HIV/STD Prevention Program aspires to be a recognized leader in the intervention of the spread of HIV/STD and to reduce the complications of these diseases. Compassionate and professional services are provided in a confidential and timely manner to include surveillance, case detection through screening, and

treatment of known cases, education, and case management.


HIV/STD Prevention Goals for 2011• Actively work with ISDH to get reporting features added to SWIMSS to support our data needs. • Advocate for system edits which make SWIMSS more user friendly and time efficient. • Provide timely HIV/STD surveillance for District III, including timely interviews and contact investigations to reduce the

transmission of HIV/STDs.• Enhance the training and development of the existing workforce. • Develop a Community HIV Case Management Care Plan.• Develop a Community Health tool kit which supports a unified, concise HIV/STD prevention message for care providers,

patients and parents. • Complete a collaborative, systematic review of the CDC’s 2010 STD Treatment Guidelines and Recommendations with the

Health Commissioner and Clinical Services staff members. Update protocols and procedures in this work group.

• Conducted immunization clinics for the mandatory teen vaccines, the month of March at Carew Street location.• Deployed strike teams to three schools to provide the mandatory teen vaccines, and to three different group homes.• Increased our Immunization Clinic for the summer to four days a week to help complete the teen vaccines for the school requirements. • Co-coordinated the Focus on Health for the City-County Building site in April.• Assisted with the hands-only CPR training for the month of May.• Functioned as a clinical site for IPFW, Ball State University and University of Saint Francis student nurses.• Co-Coordinated vaccines and information for individuals going to Haiti after the earthquake.• Partnered with community partners to provide Flu Mist in elementary schools. Highlights in 2010 • Utilized a water-based adulticide instead of an oil-based one for the night-time spraying in areas around locations with West

Nile virus-positive adult mosquitoes.• Initiated a bed bug education program for tenants and property managers of apartment complexes; A presentation was done

Highlights in 2010 • Held first quarterly meeting with all hospital birthing personnel on Sept. 29, 2010 and it continues to be a success. • Educated and assisted area funeral directors and physicians in implementing the Electronic Death Record, which was to take

effect January 1, 2011.• Honored Janet Westendorf on her retirement after 38 years of service in the Vital Records Division.

Finance

The Finance Division is responsible for the budgetary and financial functions of the Department of Health. The duties include but are not limited to the formation and implementation of the department’s budget, monitoring receipts and expenditures, grant management, and the requisitioning of goods and materials funded by the Department and grants.

The Finance Division ensures compliance by meeting internal financial reporting requirements as those set by the Allen County Council, the Allen County Board of Commissioners, the State Department of Health, and the State Board of Accounts.

The Finance Division is responsible for all personnel, payroll, attendance records, and tracking of benefits for the 55 full-time employees, 9 part-time employees and 3 people employed contractually by the department.

In support of the department’s overall mission, the Finance Division assists the department in functioning at an optimal level by

managing all financial resources.

Interns We are pleased to provide opportunities for students who are interested in gaining on-site practical work experience to enhance their knowledge and understanding of public health. Our interns for 2010 were:

Robert Pease, Healthy Homes. Robby examined demographic data (gender, race, family WIC status) blood lead levels and developmental assessment scores on lead poisoned children in Allen County to identify the impact of lead poisoning on children’s development through the age of six in each demographic group.

Jessica Trensey, Heathy Homes.Jessica created a Built Environment training manual for Department of Health staff. Jessica also assisted with creating an impact statement related to falls in the elderly population.

Kelsey Kleber, Clinical Services. Kelsey developed and conducted a survey of patients in the STD clinic to gauge their knowledge of sexually transmitted disease. The information will be used to improve outreach and educational programs related to STD prevention.

Joseph Cologne, Communications.Joey compiled information on social media and its use in health care for a PowerPoint presentation.

Kathryn Anderson, Communications/Administration.Katie created an informational brochure to help explain the basics of health insurance to indigent and refugee clients. In addition, she compiled a brief history of the department through the 1970s which is posted on the department’ s Web site.

Holli Duncan, Informatics.Holli created various GIS maps for the Pollution Control and Food & Consumer Protection divisions, as well as assisted in conducting measurements of the Department’s furniture and equipment in anticipation of a move from the City-County Building to the Renaissance Building. She was also involved in other IT projects.
