Sample Collection Standard Procedure

Notification

1- Escort identify him/herself

2 -Confirm identity of the athlete

3- Details of the notification:Specific time – not more than 1 hr

4- Consequence of failure to report

5 -Sign on notification by escort

6- Sign on notification by athlete

7 -Right to be accompanied

Escort the athlete at the time until athlete has checked in DCS

Doping Control Station

8- Check in at the registration desk In the Waiting Room of the DCS

9- Record time of arrival/

Verify identity of athlete/ Check Doping Control Pass

10- The athlete is under supervision until the whole procedure is completed and the athlete leaves the Doping Control Station.

No photos, video or tape recordings are taken inside the Doping Control Station

Working Room

11- Confirm identity of the athlete(Only one athlete at a time is allowed in the Working Room)

12- The athlete selects a sealed collection container (vessel), The athlete checks and opens it

13- DCO asks for adequate

sample

The athlete handles the

collection container

and urine sample until

the sample is sealed.

14- Only the witness and the athlete enter the toilet

15- Clothing that prevents the direct observation of the urination is removed

16- The witness signs on the Doping Control Form

17 -The athlete selects & opens a urine control kit.

18- The athlete and DCO check that bottles are empty, clean and properly sealed

19- The athlete pours at the minimum volumes of urine into bottle B &A and leaves a few drops of urine the collection vessel. Then bottles are sealed probably and check for possible leakage.

20- The DCO measures PH & SG

21-The DCO records any medication and supplements

22 -The athlete places the bottles A & B into the respective shipping containers and closes them.

23-The DCO records, if any, irregularities of the procedure in the Doping Control Form. 39-Sign on the form

24 -Sign on by Athlete and accompanying Person

40- A copy of the Doping Control Form is passed to the athlete
