Risk Bow Tie

Steps to Define Your Safety Critical Equipment (SCE)

• Define your Barriers / Categories and Owners

• Maintenance / Reporting Strategy

• Initial Identification of SCEs

• Review with Technical Authorities, Safety Engineering, Independent Verifier

• Finalize your SCEs (30-50% of your asset register)

Define your

Barriers / Categories and Owners

Escalating Consequences

- Sub sea / Foundation Structures- Topsides & / Surface Structures- Heavy Lift Cranes- Mooring Systems- Drilling Systems

- Pressure Vessels- Heat Exchangers- Rotating Equipment- Tanks- Piping Systems- Pipelines-Well Containment-- Tanker Loading

- Helicopter Refueling Equipment

- Haz. Area Ventilation- Non-Haz. Area Ventilation- Certified Electrical Equipment- Cargo Tanks Inert Gas System- Earth Bonding- Miscellaneous Ignition Control

- Fire & Gas Detection

- Deluge Systems- PFP / Fire / Explosion Protection- Helideck Foam Systems- Firewater Pumps

- Firewater Ring main

- PFP

- Nav. Aids- Collision Avoidance- ESD- HIPPS- Relief System - Ballast & Cargo Management-Chemical Injection-Power Management-Fine Water Spray-Gaseous Fire Protection

- Depressurization ESD- Well Isolation Depressurization- Pipeline Isolation Valves - ESDVs Well Isolation- SSIVs Pipeline Isolation Valves

- Drilling Well Topsides ESDVs Control Eqpt.- SSIVs

- Temporary Refuge- Escape & Evacuation Routes- Emergency / Escape Lighting- Communication Systems- UPS- Helicopter Facilities- Open Haz. / Non-Haz. Drains-Open Non-Haz. Drains- Em. Power-Manual Firefighting

- PSE- Rescue Facilities- Lifeboats / TEMPSC

- Tertiary Means of Escape

Define your

Maintenance / Reporting Strategy

What is my SCE Maintenance / Reporting Strategy

Fire and Gas Detection – Loop Level

Pipeline Isolation – Valve or Item Level

UPS – Battery or Item Level

Pressure Vessels – Item Level

Emergency Lighting – Loop or Area Level

Initial Identification of SCEs

What are my SCEs?

Must Start with a Good Asset Structure

How are these defined? EX Equipment, Structures (TS,SS), Lifeboats, Emergency Lighting, ESD

Fire and Gas Detection:

Detectors – Gas, Flame, Heat, Etc.

Defined / Maintained by Platform/Vessel, Deck, Zone, Loop, Area, Item?

Performance Standards

• Definition – Documentation of the specific measureable targets of an SCE to assure it is available and functioning to prevent or mitigate a Major Accident

• Design Vs Measurable?– The Escape Route will be 1 meter wide– The Escape Route will be free of obstruction

Performance Standards should be:Understandable and Measurable

• Technicians need to be able to pick up a PS and understand exactly what they are required to do.

• The Assurance Activities should be measurable with either a Y/N or with a calculative response. – valve closes <30 seconds– Gas Detector alarms at >10 PPM– Emergency Lighting works? Y/N

Not Understandable and Not Measurable
