• State mandated physical with updated immunizations due by September 30th
• Medication • All medication must be delivered to the nurse in the original prescription bottle or over
the counter medication packaging.
• All medication must be delivered to the nurse by an parent/guardian or arranged with the nurse for a student to deliver.
• No student may carry medication on their person
• One exception - may carry inhaler w/doctor’s note
• Physician’s notes and medical issues should be submitted/communicated directly to Mrs. Madigosky
Camp Canadensis
Important Information• Coordinators: Trish Young, Michelle D’Avella, and
Beth Mastrocola
• Dates of trip: June 1st, 2nd, and 3rd
• Camp Canadensis in Canadensis, PA
• Approximately 2.5 hours away
• Check out their website: www.canadensis.org
• Check the HMS website (6th grade page) for all forms and updated information as it becomes available
• Camp Canadensis information letter with several enclosures mailed at the beginning of October.
Purpose of Camping trip• To offer students opportunities to enrich their
personal growth.
• To transplant our 6th grade curriculum to an outdoor environment for 3 days.
• It is also a ton of fun and for a lot of our students their most memorable middle school experience!
Medical Information• Medical facility staffed 24 hours a day with
currently licensed nurses and physicians nurses and MD (provided we have a volunteer)
• Medication distribution is confidential under guidance of medical professionals
• Accommodations for food allergies
• Peanut free facility
Cost• $165 per student, no cost increase for 2016
• Includes bus, room/board, all activities
• Check or money order only
• Installments due by November 20th and February 5th
Parent Chaperones• All potential chaperones must acquire the following clearance
documents:• PA State Police Background check
• FBI Criminal Clearance
• PA Child Abuse Clearance

• All potential chaperones must fill out a school application in full.
• Potential chaperones in the medical field who can volunteer to support staff and students will be given preference.
• All other potential chaperones will be chosen through a lottery system.
• Chaperone cost, $110
Please Note…
• Students must commit to attend camp by April 1, 2016
• If your child is expressing hesitancy about the Canadensis trip, please contact Beth Mastrocola
• Discipline issues may prevent a student from attending the trip
• Check your family schedules now to avoid conflicts with the camping trip on June 1st, 2nd, and 3rd
Cabins
• Student choice of 3 friends to bunk with
• 9-12 students total in cabin
• 2-3 parent chaperones in cabin (dads with males, moms with females)
• Electricity and running water in all cabins
Activities• Randomly selected
groups of 21-24 students and 4-6 parent chaperones
• Students are encouraged to try all activities
• Examples include: boating, cooperative games, orienteering, swimming, t-shirt art, climbing “the wall,” cooking, pond study, floor hockey, bracelet art
Meals
• Many selections for breakfast, lunch and dinner
• Sensitive to all food allergies/ peanut-free facility
Band & OrchestraParents of band students will meet with Mr. Gittelman when they follow their child’s schedule tonight
Parents of orchestra students will meet with Mr. Brennan when they follow their child’s schedule tonight
