INTRODUCTIE
• 55% van alle heup fracturen• Laag energetisch trauma• Oudere patiënten:• Osteoporose• Co-morbiditeit• Hoge mortaliteit• Incidentie ↑
• Extra capsulair• Goede vascularisatie• Dislocatie risico
• Trabeculair bot
ANATOMIE
• Trochanter minor: insertie m. iliopsoas• Trochanter major: insertie van m. gluteus medius/
minimus, m. obturator externus/internus/superior, m. gemellus inferior/superior, m. piriformis
• Secundaire dislocatie bij pertrochantaire #:• Proximale fragment in abductie, flexie, exorotatie• Distale fragment naar mediaal
• Progressieve varus dislocatie

• Afhankelijk van het fractuur type:• Modificeren van de reductie• Kiezen van de fixatiemethode• Aanpassen van het post-operatieve beleid

INTRA MEDULLAIRE FIXATIE
• Toename in gebruik, evidence echter niet eenduidig• Voordelen:• IM fixatie theoretisch superieur voor neutraliseren van krachten die #
disloceren • In vitro vele malen sterker dan DHS• Minimaal invasief, bloedverlies ↓• Infectie risico ↓• Snelle revalidatie• Duidelijk superieur voor fixatie van A3 fracturen

plaatsing is afgenomen met de recente implantaten (PFNA, TFN)• Geen evidence dat i.m. implantaten superieur zijn voor de
belangrijke uitkomstmaten• Voor A3 fracturen zijn i.m. pennen geassocieerd met een betere
uitkomst
Echter: onvermogen van reviewers om onderscheid te maken tussen stabiele- en instabiele fractuurtypen

CONCLUSIE & DISCUSSIE
• De status van de laterale wand op de pre-operatieve röntgenfoto moet bepalend zijn voor het osteosynthese materiaal dat wordt gekozen:• A1- A2.1 DHS• A2.2-2.3 TFN/PFNA/PCCP• A3 TFN/PFNA
• Welk implantaat gekozen moet worden voor A2 fracturen is per casus verschillend
• Er is behoefte aan een RCT bij instabiele pertrochantaire fracturen
