,,Thanks to all of you, the Aloha Medical
Mission (AMM) experienced a record-breaking
year in 2017. Locally we have increased the
number of no-cost dental visits in Hawai‘i by
nearly 30% and delivered quality surgical and
medical care on seven overseas missions —
Comayagua in Honduras, Dawei in Myanmar/
Burma, Kabakalan in the Philppines, Lahan in
Nepal, Jinotega in Nicaragua, Homalin, Sagaing,
and Inle Lake in Myanmar, and Dinajpur in
Bangladesh. We have worked closely with
foundations to change lives through our Welcome
Smile program as well as provided prostheses to
patients in other countries, who lost hands due
to poor nutrition or lack of medical attention,
enabling positive life-changing opportunities.
It was not easy. It required hard work, dedication
and perseverance from more than 16 volunteer
dentists along with our devoted staff and loyal
board of directors, as well as countless mission
volunteers from all over the world. Together,
we can proudly proclaim that we brought
hope and changed the lives of the people in
Hawai‘i and beyond.
One project we are especially proud
of, links the AMM with Hawai‘i’s hospital emer-
gency departments (ED) to establish a referral
partnership. When patients go to the ED for
non-traumatic dental conditions (NTDCs), they are
referred to the AMM so that we can provide care
for them. The goal is to fix their dental problem in
the appropriate setting to reduce the number of
NTDC patients who return to the ED. This will save
hospitals and the state millions of dollars.
We raised the bar in 2017 with tremendous growth
and expansion. Yet, we are excited about the
future and are looking forward to even more growth
in 2018 and beyond. We are committed to helping
global communities as well as families in Hawai‘i. Our
desire is simply to make sure that those who need
care receive the help they need.
We would like to give a heartfelt mahalo to the
generous foundations, organizations and devoted
individual donors who believe in our mission and
provide their ongoing support. We have taken stew-
ardship of the funds entrusted to us very seriously.
Our disciplined approach to financial management
continues to earn us exemplary ratings from several
auditing agencies. Our strong financial performance
with solid operating revenue and a growing patient
base provide us a solid foundation on which to
build and we hope each of you will continue to sup-
port our efforts as the AMM moves forward.

GLOBAL SURGICAL MISSIONS The medical-surgical teams sent to the developing countries
of Asia and the Pacific are a cornerstone of Aloha Medical
Mission’s activities. The first missions occurred in the early
1970s, when Dr. Ernesto Espaldon, a plastic surgeon from the
Philippines then living in Guam, and his anesthesiologist wife
returned to his hometown of Tawi Tawi in Mindanao to fix the
cleft lips of children. The Aloha Medical Mission was formed
when Hawai‘i physicians teamed with Dr. Espaldon in 1983 to
assist him in his efforts.
Our surgically focused missions are composed of general
surgeons, gynecologists, dentists, anesthesiologists (includ-
ing CRNA’s), and plastic surgeons. Other specialties (eg ENT,
ophthalmology) are sometimes represented. Medical spe-
cialties (family practice, ER, internal medicine, pediatrics) are
included depending on the location and local needs. Nurses
are an essential component of the mission team and include
these specialties: OR, ICU, recovery, ER, med-surg, Nurse
Practitioners/Assistants, Dental Assistants and OR Technicians
are also needed. Uro-gynecology subspecialists are needed
to treat uterine prolapse. Mission sites are generally in small
rural towns where the population is most frequently medi-
cally underserved.
HAWAI‘I’S ONLY NO-COST DENTAL CLINICHawai‘i’s mission began in 1995 at the Institute for Human Services
(IHS) as a free medical and dental clinic providing basic services
on an interim basis. In 2002, the AMM relocated to Palama
Settlement, once the home of the Strong-Carter Dental Clinic,
under a lease terminating in 2021. In Hawai‘i there are substantial
dental health disparities among low income families, which
compelled AMM to become a safety net for the underprivileged.
Goals are to improve overall oral health awareness in the
community, develop oral health programs and services, and
collaborate with community partners and allied professions to
promote oral health prevention and treatment. Through the
VISTA/Americorps program, our administrative intern compiled
data to help visualize the clinic’s growth and capacity as well as
reconnect with community stakeholders.
In 2017, under the direction of the AMM’s dental director Dr.
Mendel Sato, four paid clinic staff and sixteen volunteer den-
tists, the interim clinic continues to be a success by performing
basic dental procedures such as examinations, cleanings, fillings,
extractions and selective root canals for qualified low-income
individuals and families all across Oahu. As the last safety-net for
dental care, the AMM grows in patient numbers with anticipation
of further growth as more learn of AMM’s no-cost dental services.

PATIENT GROWTH REQUIRES PATIENCE IN HEALTH CARE
MAKING AN IMPACT Since 2002, AMM dental services have
reached across Honolulu county. From
2014 to 2017 out of 2,264 patients
served, 23.6% (481) of our patients came
from Leeward Oahu, including Waianae;
15% (309) from Central Oahu, including
Wahiawa; and 8.6% (176) from Wind-
ward Oahu, including Laie. However,
the largest majority 52.5% (1,068) came
from Honolulu, which includes the Kalihi
Palama area because of the easy acces-
sibility of the clinic to the surrounding
community, including public housing.
The assumption is if we conduct Teach
& Reach screenings to nursing students
who already treat patients in rural
locations where houseless individuals
reside, we will begin to target families
and individuals who would otherwise be
unaware about AMM’s free services. We
will then begin to have greater impact
through education when in collaboration
with students during routine medical
check-up/screening.

First Smile Dental Education

First Smile is Aloha Medical Mission’s (AMM) oral health education program for pre-school
through third grade students. The goal is to reach children at an earlier age to prevent
dental complications as adults. AMM is partnering with neighboring public elementary
schools, HCAP Head Start pre-school classes and other non-profit programs. Schools have
the opportunity to tour the AMM Dental Clinic or have an educational session in the class-
room. Each child receives an activity bag with a child’s toothbrush and toothpaste, simple
instructions on how to brush properly and AMM Clinic information for parents.
Welcome Smile is a restorative dentistry program designed to help women who have
experienced abuse or incarceration. Typical dental concerns include cleanings, fillings,
extractions, missing teeth, broken or cracked teeth, and discolored or decaying teeth.
Patients are provided with temporary prosthetic pieces that will help to build their confi-
dence to may lead healthy and productive lives geared for reentry into the workforce.

MAKING AN IMPACT In 2017, 11 missions have served
the indigent in Myanmar (4), the
Philippines (3), Central America (2),
India, and Nepal. These patients would
otherwise never have received the
life-changing surgical attention had
AMM not visited their town. The AMM
has performed more than 2,100 major
surgeries (gallbladders, hysterectomies,
thyroidectomies, hernias), 5,200 dental
procedures (extractions and fillings),
600 minor surgeries, seen 7,000
medical patients, and provided 100
free hand prostheses to amputees:
All doctors and staff covered their
travel with no fees charged to patients.
This equates to more than 17 million
US dollars of in-kind surgical and
medical services.
ALOHA SOCIAL SERVICES BANGLADESH (ASSB) is a
year-round ongoing project that began
in 1998 as a windowless, one-room clinic
with a table for an exam bed. Named
ASSB by the sponsoring Bangladesh
family, this branch of the Aloha Medical
Mission has expanded over the years
to meet the multiple needs of the local
indigent population.
A boy who had a burn contracture released by an AMM plas-tic surgeon (Dhankuta, Nepal), and the result 2 years later. The dad was so grateful, he took a full day and a half to travel from his town to visit the AMM Surgical Team (on a return mission 2 years later) to show how well his son had done since the surgery.
