Animal Health Network Information Sheet for Agents

1. Recruit at least one feed retailer in your county to participate in the Animal Health Network

a. Marketing materials are available to provide more information to the feed retailers

2. Collect information from participating feed retailers:a. Collect contact information from feed retailer owner/manager. You may

want to tell them that UK CES will be holding their information and that we are a non-regulatory agency.

i. Preferred method of contact1. Do you want to give messages to the feed retailer?2. Do you want Ag Programs to give messages to the feed

b. Collect information about how the message will be promoted to the consumers of the feed retailer (i.e. how will the consumers receive the message – displayed on a bulletin board, flyer, word of mouth from cashier, etc.).

c. Provide additional demographics for your county:i. Indicate how many total feed retailers are in your countyii. Describe why you selected the feed retailer(s) you did to target

for participation in the program3. You will be provided a link to Qualtrics to submit this information.
