For: Minor Building Repairs for Long Term Care Homes Please refer to the above Request for Quotations (RFQ) document in your possession and be
advised of the following:
I. Revisions, Additions & Deletions 1. Description change on page 1 and page 4
From: Minor Building Repairs To: Minor Building Maintenance and State of Good Repairs
2. Add the following paragraph to page 4 as Article 1.2:
1.2 The City of Toronto, Long Term Care Homes and Services Division (LTCH&S) has 10 Long Term Care Homes in the GTA. These LTCH&S 10 sites provide excellence in long-term care to over 2641 residents. The LTCH&S individual Homes are located throughout the City of Toronto and are multi-story structures in various ABC classifications ranging in age from one year to over 50. The LTCH&S Homes range in size from 56,000 to 246,000 square feet for a total area of approximately 1,600,000 sq. ft. This prequalification will involve projects at the following locations:

3. Add the following definitions to Section 2.0 Terminology:
"IP" means Inspection Protocol that is used by MOHLTC Inspectors
"IPAC" means Infection Prevention and Control Policy and Procedures of LTCHS Division
'LTCHS" Long Term Care Homes and Service Division of the City of Toronto

"minor building routine maintenance and state of good repairs" means building maintenance work performed in order to prevent the normal deterioration of the land, building, structure or work or to maintain the land, building, structure or works in a normal, functional state.
'MOHLTC" means Ministry of Health and Long Term Care "Orientation" means contractors orientation before starting the contract work
"PIDAC" means Provincial Infectious Diseases Advisory Committee
"Resident bill of Rights" means the Resident Rights who live in the Ontario Long term Care
Homes
4. Replace the Scope of Work on page 9 with the following:
"Minor building maintenance and state of good repairs shall include, but not limited to dry
wall, plastering, sheet floor, epoxy flooring, floor tile, wall tile, ceiling tile, masonry,
painting, and minor carpentry works, on an “as required basis” for the period of a one (1)
year, with options to renew for a further 4 additional one (1) year periods, as directed by
the Manager Environmental Services, Long-Term Care Homes and Services. All work
must be carried out expediently and within a safe working environment. "
5. Replace Section 6.7 Bidder References on page 12 as follows:
"6.7.1 In order for the City of Toronto to confirm that the Bidders or their sub-contractor(s) have experience in providing services detailed in this RFQ to similar health organization, Bidder must submit at least three (3) references for Long Term Care or Acute Care Services in a facility reference. References must be submitted with the Bid Submission at the date and time of Call Document closing."
"6.7.2 The Long Term Care references must meet the following minimum requirements…"
"6.7.4 Bidders are requested to submit a minimum of three (3) references from companies, for whom the Bidder has satisfactorily provided Minor building maintenance and state of good repairs in the past two (2) years in Long Term Care, acute care facility."
6. On page 13, replace "Minor Building Repairs" with "Minor Building Maintenance and State
of Good Repairs"
7. Replace entire Section 7.0 Evaluation of Bids with the following:
"7.1 Subject to the process terms contained in Appendix "A" and the City's procurement
policies, it is the intent of the City to award a Contract to one (1) or more Bidders based
on the Bidder meeting specification and providing the lowest grand total as shown in the
Price Form.
7.2 The successful bidders will be assigned one of the two district groupings. District "North"
will consist of Etobicoke North, Willowdale and Scarborough East City Wide. District
"South" will consist of Etobicoke South, Etobicoke Lakeshore, Toronto South and Beaches
East York. Please refer to enclosed Long Term Care Homes Boundary Map. The City
reserves the right to have the successful bidder perform services in alternative Districts if
applicable.

7.3 The City reserves the right to award this contract in its entirety or to split the award by
geographical areas (Districts) at the City's discretion. Upon award, a purchase order shall
be issued to the Successful Bidder(s). Such Purchase Order shall incorporate or deemed
to be incorporate the Contract Terms and all other provisions contained in the RFQ.
7.4 The City reserves the right of accepting or rejecting any and/or all parts of this RFQ.
7.5 Upon award the City will confirm with the Successful Bidder(s), the products and/or
services to be provided and any other instructions. The provisions of the Products and/or
Services shall not commence until a signed Contract for the products and/or Services is
issued and a Contract Release Order (CRO) has been issued.
7.6 The decision to renew the contract for the option year(s) will be at the discretion of the
City. Included in the decision to accept option year(s) of the contract is a market analysis,
conducted by the City for goods/services included herein, at the time the option is being
considered, comparing the bidder's change in price for the option year(s) to current
market/industry conditions."
8. Replace Article 9.6 on pages 14 and 15 with the following:
"9.6 This work may require, minor building maintenance and state of good repair, which will
include the repairs of drywall partitions, the repairs of damaged surfaces, repairs of doors,
windows, etc. The supply of materials shall be part of the requirement. The completion
of this work will require close co-ordination to avoid disruption of ongoing operations."
9. Replace "building repairs" on page 16, Article 11.7 with "minor building maintenance and
state of good repairs".
10. Replace Articles 11.13 and 11.14 on page 16 with the following:
"11.13 In case of a dispute in reference to labour hours charged, the Successful Bidder will be required to show proof of staff labour hour time sheets which will be cross referenced with the Contractors Sign/Out Signing Sheets which every contractor trades person must sign in/out, and evidence of work performed over and above the amount originally stated. All additional labour is to be approved by the Owner’s site management personnel before being performed.
11.14 Labour rates to include material, accessory cost, tools, mileage, vehicle and equipment
charges and travel time, over-head and profit." 11. Replace entire Article 22.1 on page 24 with the following: "22.1 Unless stated otherwise in the specifications the vendor shall offer performance warranty
of all materials, labour and quality of work for not less than two (2) full years after completion and acceptance of the work."
12. Replace the entire 2nd paragraph under Article 20. City of Toronto Invoice/Billing
Requirements on page 43 with the following paragraphs: "20.2 It is the Vendor's responsibility to submit accurate invoices for goods /services delivered
to the City of Toronto divisions. This invoice must be submitted along with the detailed work order, the work order must indicate labour hours for each trade and the material used for the work. The successful bidder(s) must obtain the signature of the Building Service Manager/Supervisor for the location on the work order verify that the work was

completed and satisfactory. A copy of the work order to be attached with the invoice before it is sent to Accounts Payable.
20.3 The successful bidder must be prepared to provide to the City proof of the "Trade Cost"
within five (5) business days from request. Specifically, a copy of their trade invoice as well as the corresponding copy of the invoice issued to the City for any materials specified by the City to validate that the mark-up structure applied by the supplier complies with the mark up structure detailed in the bid document. Failure to comply with this request, or in event the mark-up structure is not in accordance with what was provided in the bid document, will provide cause for the City to terminate the business arrangement with the successful bidder.
20.4 If an inaccurate invoice (ie: but not limited to, pricing, quantity or services provided) is
submitted, the Vendor will be requested to issue a credit note and submit a new invoice. If the invoice in question offered an early payment discount, the re-issue date of the new invoice will be used to calculate the early payment discount terms."
13. Replace the entire Schedule "A" Price Form with the attached Revised Schedule "A". Failure to use this revised price form will render your bid non-compliant. II. Attachments

REPAIRS, EXCLUDING HARMONIZED SALES TAX (HST)
IN THE EVENT OF MATHEMATICAL ERRORS FOUND IN THE PRICING PAGES, THE UNIT PRICES
QUOTED SHALL PREVAIL. EXTENSIONS AND TOTALS WILL BE CORRECTED ACCORDINGLY AND
ADJUSTMENTS RESULTING FROM THE CORRECTION WILL BE APPLIED TO THE TOTAL PRICE
QUOTED.

All prices and percentage (%) line items are to be filled in. Any omission in completion
of any/all Price Form(s) will result in the bid submission being declared non-compliant.

intended to be zero cost/ no charge to the City of Toronto are to be submitted in the

Note:

MARKUP ON MISCELLANEOUS MATERIALS
Price for all parts and material (if required)
Parts and material are to be provided and charged at "TRADE COST" plus 10% mark-
up for overhead, profit and all other charges. Trade cost shall mean the actual cost to
YOUR ORGANIZATION after deducting all trade discounts. Do not include Harmonized
Sales Tax (HST) in your pricing.
The Bidder must provide the percentage mark-up on Material over "Trade Cost" in Line
(Y) on the Price Schedule. The percentage mark-up shall remain the same for any/all
Contract term(s).
The Successful Bidder must be prepared to provide to the City of Toronto proof of
"Trade Cost" within five (5) business days from request. Specifically, a copy of their
trade invoice, as well as their corresponding copy of the invoice issued to the City of
Toronto, for any item or service specified by the City of Toronto to validate that mark-up
structure applied by the suppliers complies with the mark-up structure detailed in their
Bid. Failure to comply with this request and/or in the event the mark-up structure is not
accordance with what was provided in the Bid, will provide cause for the City of Toronto
to terminate the Contract with the Successful Bidder.
SERVICE CALL MINIMUM CHARGE:
