There was crushing knee of the left leg;

Several exhibited extremely serious and fractures in the same leg;

Burns 1 and 2. Degree in your hands, arms and chest.

The accident not only had greater consequences because:

Realizing that the table was walking toward me. I jumped on the mandrel hot because was been processed coil lately (resulted burns).

If I had not done this, the table would have crushed my chest;

Another employee acted quickly by placing a metal tube between the table and the mandrel, thereby preventing my leg was guillotined by threading table;

Causes of the accident

Lack of a more efficient risk analysis for task execution in a technically safe way;

Lack of electric locking of all mobile equipment that could cause accidents if triggered.

Lack of mechanical lock of all mobile equipment that could cause accidents if triggered.

Lack of communication between everyone. of diverse, complex and different services running in the same workspace;

Lack of attention and care throughout the process of implementation of the tasks for this sector in particular;

Failure to follow standards and regulations adopted by the company to performing this task;

Overconfidence on the task performed under high degree of unsafe conditions;

ACCIDENT

- Use personal protective + nominees for service equipment to run;

- Acting with care and attention throughout the execution process .

- Avoid committing unsafe act ( malpractice , negligence and / or recklessness ) ;

- Always try to eliminate or control the situation Unsafe Condition; - Make risk analysis (Look get this habit).

- Do not improvise.

- Disconnect and lock the equipment that interfere with the action.

- Clean and organize the site after execution of the service.

- Maintain effective communication (Always confirm any policy or guideline to be not mistake when you run the service).

- Always follow the standards and procedures adopted in the company for the service.

- Trust no one, much less in locked equipment without first testing the shutdown and even mechanical and electrical lock.
