Thorakoskopie und Pleuraerguss
Warum thorakoskopieren?Klärungsrate > 90%gute Histologie bei großen Biopsien
zusätzlich Hormonrezeptorbestimmungzusätzlich Immunhistologie
früherer Diagnosezeitpunktexakteres Staging, auch präoperativAblassen größerer Mengen problemlos
mehrere Liter meist ohne Re‐ExpansionsödemBeurteilung der Dehnungsfähigkeit (gefesselt?)
Option zur therapeutischen Intervention
Why you do or do not need thoracoscopyJ.P. Janssen, Eur Respir Rev 2010; 19: 117, 213–216
The diagnostic yield of pleural effusions under CT or US guidance is only slightly inferior to thoracoscopy (85% versus 90%).
Thoracoscopy is the preferred procedure if no clear target lesion is visible on the CT scan.
Also, thoracoscopy is preferred if removal of pleural effusion and/or pleurodesis is indicated.

PleurodeseZiel
• Prädiktor: pH‐Wert (< 7,20) [~40% sprechen dennoch an!]
Nicht entfaltete Lunge beim Pneumothorax• Fistel bei expandierter Lunge KEINE Kontraindikation!
Geringe Lebenserwartung• fortgeschrittene Stadien besser wiederholte Punktionen oder PleurX‐Katheter (getunnelte Drainage)
TalkumpleurodeseMechanismus
Ziel‐ und Schlüsselzelle: MesothelzelleErstes Ereignis: „Schädigung“ der PleuraEinsetzen eines multifaktoriellen Prozesses
• unspezifische gewebetoxische Effekte• substanzspezifische Effekte
• akute granulozytäre Phase, stimulierte Makrophagen• proliferative Phase
definitive Fibrose• Verklebung des Pleuraspaltes

Thorakoskopische TalkumpleurodeseTechnik I
Fa. K. Storz, Tuttlingen)• große Magenspritze und Drainageschlauch• Talkumspray (Steritalc/Mucosol®, Fa. Bess, Berlin)
Talkumpulver – Zerstäuber (Storz)
PleurodeseTalkumpoudrage
Thorakoskopische TalkumpleurodeseTechnik II
Einlage von 1 (‐ 2) Drainagen• Ablaufen des nachgebildeten Exsudates
• Kontakt der Pleurablätter essentiell
üblicher Dauersog 15 (‐ 20) cm H2O
Thorakoskopische TalkumpleurodeseTechnik III
Entfernung der Drainage(n), wenn• die Lunge anliegt (Röntgen‐Übersicht)• die Nachproduktion < 100 (50) ml/d • Durchgängigkeit der Drainage(n) prüfen!
ausreichende Schmerzmedikationbegleitende antibiotische Therapie (??)
• Cefazolin i.v.*
Nachgeben einer Aufschwemmung möglich• üblicher Zeitraum: 3 ‐ 4 (‐ 6) Tage• Einbringen von 3 ‐ 5 g „Talc Slurry“, gegebenenfalls wiederholt (Obergrenze?)

Komplettes Ansprechen• keine signifikante Ergußansammlungwährend des gesamten Follow‐up überprüft durch Röntgen/Sono
• Gesamtrate: ca. 89 %*• pH > 7,30: 95 % der Fälle*• pH < 7,30: 76 % der Fälle*
• auch bei pH < 7,30: 88 %#
Kein Ansprechen: ca. 10 %#+(meist gefesselte Lunge)

PurposeDetermination of long‐term efficacy and safety of pleurodesis by thoracoscopic talc poudrage (TTP) in malignant pleural effusions (MPEs)
MethodsSeries of 102 patients (45 women, 57 men; 20 to 83 years of age), who underwent medical thoracoscopy and TTP for recurrent MPE between 1999 and 2001
Medical Thoracoscopy (Local anesthesia, IV sedation)Pleurodesis (8 g of sterile talc powder)Follow‐up
180 days with outcome measures• Time to recurrence of the effusion and survival
Efficacy was judged by clinical examination, chest radiograph, and/or thoracic ultrasound examination
