The Venous sysTem

all blood returns to the heart through the veins in our body. since the legs are the

farthest part of the body from the heart and are below the heart, the column of blood

in those veins exerts a considerable downward force. It follows, that even more pressure is

needed to move that blood back to the heart.

The venous system has a special mechanism for propelling blood toward the heart. It is

referred to as the “venous pump” or “muscle pump”.

all leg veins contain valves,

which allow blood to flow only

in the direction of the heart. They

are located about every inch and

a half. When we contract our leg

muscles, as in walking, the blood

is squeezed through the veins,

toward the heart, because of

the valves.

When these valves no longer

function as intended, it causes

increased venous pressure. The

column of blood is no longer

broken into segments by the

valves. This could be due to

inactivity, trauma, disease or

pregnancy. over time, it can lead to chronic leg fatigue and swelling in the lower limbs.

When these valves fail in the superficial system, the increased pressure can lead to

varicose veins. It can also lead to more serious problems when they fail in the deep veins

of the leg. Unchecked, chronic venous insufficiency can lead to leg discoloration, stasis

dermatitis, venous ulcers and deep vein thrombosis.

Compression hosiery

The role of compression hosiery may be as a preventative measure or as a treatment

option for active venous disease. When used as a treatment option, an adequate

compression level and compliance with the compression therapy can halt the progression

of venous disease.

Compression hosiery accomplishes this by applying gradient compression, tightest at the

ankle and decreasing in pressure as you move up the leg. This compression is normally

expressed in mmHg (millimeters of mercury). The application of compression hosiery can

benefit the wearer in several ways. by increasing the pressure in the leg, fluid leakage out

of the capillary beds is reduced and absorption of fluid by both the capillaries and the

lymphatic system is increased. This action may reduce or prevent swelling of the lower

limb. by reducing the size of the superficial veins, overdialation with blood is prevented.

This causes blood to flow more quickly into the deep veins of the leg. finally, the

compression acts as an assist to the muscle pump of the legs in propelling blood through

the leg and back to the heart.

Compression hosiery

selecting the right compression therapy is critical for successful treatment of venous

problems. several factors need to be considered.

Compression level – This is usually determined by your doctor or by using an indication

Guide based on the venous condition.

fit – fit is extremely important. It is the one thing that determines the accuracy of the

compression level. Make sure you fit within all of the sizing parameters of the

garment selected.

style – Determine how much of the leg needs to be

covered by the garment. Considerations would include

the height of varicosities, severity of the condition, fashion

issues, comfort and compliance.

Compliance – It is important that compression hosiery be

worn every day. new wearers should acclimate there legs

to the pressure by extending the wear time each day, working

up to a full day after about a week. It is also recommended

that you have at least two pair of hosiery to rotate. oils from

the skin need to be washed out daily and the hosiery should

rest for a day so it can return to its original shape and deliver

consistent compression over time.
