Introduction This PPI Action Plan for the Disability Division outlines the key actions that will be taken to enhance Personal and Public Involvement during 2016/2017 and will feed into the corporate Trust PPI Action Plan.

2 Service User and Carer Involvement

Review and modernisation of bed based short break provision (Respite) for individuals with a learning disability and their carers

Continue to support development of LD Carers Forum as part of the overall review and modernization of short break provision

Provide feedback on outcomes of focus groups, options appraisal group and an up-date on the new model developed

Explore development of quarterly newsletter aimed at ensuring consistency and accuracy of information received by carers.

Carers Forum remains a strong influence in the development of services for individuals with a Learning Disability. Members have been involved in the options appraisal for short breaks as well as the development of a scoring tool to help determine the amount of short break eligibility. Several members have been involved in a working group to develop a scoring tool to help staff and carers to reach an objective agreement as to the number of bed based nights a carer would qualify for as part of their respite package. The Carers Forum meets monthly and receives regular updates on short breaks, day opportunities and transition. An evaluation of the Forum to see how improvements can be developed and implemented commencing 8.2.17 with Clinical Leadership Solutions. The options appraisal for short breaks completed and all Carers of individuals with a Learning Disability have been informed of the proposed model for bed based short breaks. The Specification for the proposed bed based respite is currently with DLS and will be going out to Tender within the next few weeks. Carers have also been involved in a work shop to help determine future spend for improving services to older carers of individuals with a Learning Disability. Forum members involved in selection of an organisation to provide awareness training to Carers across the Trust iro rights and entitlements for Carers of individuals with a Learning Disability. Forum members planning the development of the newsletter. PPI team member has recently met with carers and will advise on the development of media aimed at reaching a wider audience of carers.

The vision forum meets 3 -4 times per year. Attendee numbers range from 12-15 people. Bi annual news and views newsletter circulated reaching 800+service users. Minutes and newsletter provided in range of accessible formats. Range of consultations with Deaf and Hard of Hearing people iro new sign language partnership framework draft sign language Bill. There is no formal hearing Forum, but regular (monthly) consultations with the deaf and hard of hearing community in their own locality and setting are held. A quarterly newsletter “Sign of the times” is distributed to approx. 110 Profoundly Deaf sign language users in the Southern Trust

Hold focus group with Floating Support service users to gather their views and experience to help improve services

Choice to receive care in ‘home’ environment reducing stigma associated with hospital admission. Tangible support for carers. Referrals to the Behaviour Support Team increase significantly year

on year however early intervention by the Behaviour Support Team

has enabled Carers and Trust staff to manage many complex

behaviours in a more timely and effective manner. Earlier

intervention has resulted in fewer family breakdowns and less

requests for hospital admissions and short break support

Develop HT Service to support SU’s & C’s at home/ placement for 48 hours preventing hospital admission and facilitating earlier discharge

Service developed with a range of home treatment interventions to assist with prevention of admission and earlier discharge. An average of 8 people being supported 7 days on an ongoing basis. Feedback has been very positive and the service has proved invaluable at the transition phase to assist with management of challenging behaviour and intervention. Carers have also provided potential feedback at Transition Consultation re: the assurance of having someone available at the end of a phone when other services are closed.

Supported Living Services

Review of Supported Living Services

Continue to chair Steering Group for modernisation of the overall service.

Continue to support work streams and sub groups established comprising all stakeholders to enhance communication, review policies and procedures, provide clarity on staff roles and responsibilities and complete a workforce planning exercise.

Staff are considering using 10 000 Voices as a means of rating satisfaction with services.

Staff to consider how to better capture user views within Day Opportunities and also meet RQIA requirements in terms of capturing user experience, albeit in a more creative way than the annual questionnaire, which users and carers have said is the same thing every year. Staff will explore the possibility of using observations as one way of capturing qualitative data.

Engagement sessions with 284 carers and service users in attendance. A report of proceedings capturing the views articulated by service users and carers was compiled and circulated to attendees.

The feedback has also been taken account of in drafting specification for brokerage service

A bi monthly update is given to the carers forum on development of day opportunities and also feedback from carers used to shape the future development of services.

A day opps newsletter has been produced by service users also detailing developments in day opportunities. The first edition has recently been circulated.

There is ongoing engagement with service users who attend day care centres through service user meetings. Annual Satisfaction surveys conducted with service users and carers and annual reports providing feedback in regard to survey outcomes. Information via letters and meetings with service users relating to relocation to modernised Day Centres,ie Linenbridge & Manor Centre. Service user evaluation forms were distributed/ returned in Linenbridge for comments on new day Centre.

Day Care Services and Day Time Opportunities

RQIA questionnaires are distributed to service users and carers at inspections. Staff will continue to enable the carer to advocate for and explain their relatives’ needs at initial assessment meetings.

Carer’s perspective forms an important part of initial assessment re challenging behaviours. Service user and carers’ views are also actively sought and noted during monthly monitoring visits.

Accessing local amenities

Continue to support service users and their carers to ensure they are an integral part of community and voluntary services in their locality.

Launch “My Day –My Way “

Use Posters & DVD to promote Day Opportunities service - ongoing launch of dvd to be agreed when day opps coordinator returns from sick leave

Sensory Disability Services have appointed a full time community access worker and significant developments have been made to enable profoundly deaf people to access their local communities. Sensory Services continue to work in partnership with RNIB to improve access to local community services for blind and partially sighted people and this involves extensive awareness raising about the needs of partially sighted and blind people. The Sensory Disability Services have developed a pen drive for Deaf people to study for the driving theory test in sign language. This is now available on Southerni.

3 Training

Service Users who are either deaf or blind are extensively involved in awareness training of staff across a wide range of programmes. This is particularly important to staff as they have a much clearer insight into the impact of a sensory condition on a patient/client. Carers of transitioning service users with complex behaviour support needs assisting with providing training for staff. Very positive response from staff and carers. The Adult Disability Transition Team has involved users and carers in the planning of services and also in the delivery of bespoke training
