The ACE Program Number One Again! As you may be aware, the ACE program has won its second major award after winning the “Building Partnerships” award at the Hunter New England Health’s Excellence Awards. This follows its win at the Australian Doctor Medicare Local Innovator of the Year competition earlier in the year. Hunter Medicare Local Chief Executive John Baillie stated: “ This program is a very clear and demonstrable example of how a whole of system approach to an issue can yield great results and outcomes for the people living in our communities.....As someone who has a family member living in a residential aged care facility, I can attest to the fact that the ACE program provides significant peace of mind that my relative has access to a program that helps the staff caring for him manage non-life threatening acute care issues”. Congratulations to everyone who have been instrumental in developing and making this program work. ISSUE 5 October 2014 The ACE Service Gazette Newcastle & Lake Macquarie ACE Interagency Meeting 30 October 2014 The ACE Interagency Meeting is an opportunity to learn from each other and showcase how the service is making a difference. At the meeting, guest speakers present their experiences with the ACE service, and strategies for the future of the service are discussed. The next ACE Interagency Meeting will be held on 30 October, 2014 at SummitCare Wallsend Residential Aged Care Facility. The program is as follows: •10:30am-11am: Morning Tea / Site Tour •11am-12:30pm: ACE Interagency Meeting •12:30pm: Lunch and Networking Anyone who is interested in ACE is welcome to attend. To confirm your attendance contact Kelly Thomas at [email protected] ACE Feedback Loop We would like to encourage information sharing and communication between ACE partners. To do this we have developed a feedback loop. If you have a complaint or need to report a clinical incident, visit: http://bit.ly/1zWh4I6 ACE Roll Out & Site Visits We have now been to visit 99% of all Residential Aged Care Facilities in the Newcastle, Lake Mac- quarie and Port Stephens LGA to deliver manuals and provide additional ACE assistance. The ACE service is currently being rolled out to Singleton and Tomaree Community Hospital. If you would like more information, ISBAR materials or a site visit, please contact us at [email protected] ACE Phone Calls We have seen an increase in ACE phone calls over the past few months.The below graph shows 60 calls were made to the ACE 1300 phone line in January 2014, and 225 recorded in August 2014. Well done RACFs!! Upcoming ACE Education ACE education is designed to improve clinical handover, and communications between staff providing care for residents in RACFs. The training has received a very positive response from RACFs & ED staff. The evaluation data shows that 98% of RACF & ED managers were either satisfied or very satisfied with the training, and 93% reported the learning objectives were achieved. For Train the Trainer, 97% of attendees were either satisfied or very satisfied with the training, and 95% reported the learning objectives were achieved.
