HOME BASED SERVICES

Community Aged Care Package ProgramCommunity Aged Care Packages (CACPs) are individually planned and coordinated care packages, designed to keep frail and older people in their own homes. Our CACP service operates seven days per week, including on-call, 24 hour emergency assistance. We tailor our services to meet the individual needs of our clients. Services can include:

• Medication supervision • Shopping and banking

• Transportation • Socialisation

• Light housework • Assistance with personal hygiene

• Light gardening/maintenance

• Support equipment pool

• Laundry

The program is subsidised by the Australian Government. Small daily fees are payable for our services, however we will not refuse help because of an inability to pay.

To receive a CACP you must first be assessed by your local Aged Care Assessment Team (ACAT) and require at least low level care.

Veterans’ Home Care ServicesVeterans’ Home Care (VHC) is a Department of Veterans’ Affairs (DVA) program, designed to assist those veterans’ and war widows or widowers who wish to continue living at home, but need some practical help.

Our services include domestic assistance and respite care. A small fee is payable, which is determined by the DVA.

To be eligible for VHC you must be assessed by the DVA as needing home care assistance. The period and amount of care provided is assessed on an individual basis in accordance with set criteria.

Extended Aged Care atHome Dementia PackageExtended Aged Care at Home Dementia (EACH-D) packages provide high levels of support to those with dementia to assist them to remain living in their own homes.

We provide an individually tailored program to meet each of our client’s needs. Our services can include registered nursing care, personal care, transportation, social support and domestic assistance.

To receive an EACH-D package, you must be assessed by the Aged Care Assessment Team (ACAT)

The packages are subsidised by the Commonwealth Department of Health and Ageing, and a daily fee is payable.

Home Based ServicesActivity CentreOur clients have a great time in our modern purpose-built Activity Centre in Georges Hall every Wednesday and Friday.

The Activity Centre allows our clients to socialise, share a meal with friends, be entertained and take part in a wide range of activities offered every week. Our monthly bus outings are a great day out.

We pick up our clients from their own homes, provide a tasty and nutritious lunch and drop them off home at the end of the day.
