Additional comments: - 21,22,31,32 edentulous ridge ฟนหายไปจากอบตเหตลมเมอ 5 ปกอน (พ.ศ.2544) และท าฟนปลอม RPD ซ 21,22 และ

RPD ซ 31,32 ใสมาประมาณ 3 เดอน และปจจบนไมไดใส ท าใหฟนปลอมใสไมไดแลว - 15(D),16(M),25(D),26(M),35(D),36(M),45(D),46(M) มการกรอฟนเพอท า rest ของ RPD ทใหญ และมรอยผฟน ยกเวน

15,45 - 41 ฟนหกทะลโพรงฟน โยกระดบ 1 เคาะไมเจบ ไมตอบสนองตอ EPT - 42 ฟนหกไมทะลโพรงฟน โยกระดบ 1เคาะไมเจบ ตอบสนองตอ EPT +30(control ทซ43=+20) - สภาพเหงอกโดยทวไป พบมเหงอกรนบรเวณ 41 ดาน buccal 2 mm. และมเหงอกบวมแดงม plaque และหนปนบรเวณฟน

หลง PI=54,BP=24.10%

พบ widening PDL space บรเวณปลายรากฟน พบม horizontal bone loss ระดบ cervical 1/3

1. Systemic phase :

Physician consultation : Epilepsy from post-traumatic encephalomalescia สามารถท าฟนได

แตใหระวงเรองอาการชกขณะท าหตถการ เนองจากผปวยไมมอาการชกแบบเกรง และมอาการ

เตอน หนาว ขนลก อาจใหยากนชกทานตอนเชากอนท าหตถการ

3.1 Preventive treatment : OHI โดย สอนแปรงฟนดวย Modified Bass’ technique สอนการใชไหม ขดฟน ใหhome fluoride (0.05% NaF) และ topical fluoride (2%NaF)

3.2 Periodontal treatment : Scaling and root planing full mouth

3.3 Operative treatment : 11(M,D),16(L),17(O),26(O,L),27(O),36(O,B),37(O,B),46(O,B)
