2. 02/15/10 Equilibration in FSCP 3. 03/02/10 after equilibrating the patient, the crowns on teeth #4 &

5 come off when placing the rubber dam clamp to isolate teeth #6-11 for incisal composites. The teeth had recurrent caries under the restorations that was not evident on the radiographs. The caries was removed, the crowns were re-cemented with a resin glass ionomer cement until a final treatment decision was made .
