Hokett, DVM Best Friends Animal Hospital 4175 East Mission Blvd Fayetteville, AR 72703-4388 RE: Best Friends Animal Hospital AFIN: 72-01633 Permit No.: 4864-W Dear Dr. Hokett: On May 6, 2009, I performed a routine compliance inspection of the waste treatment and disposal facility in accordance with the provisions of the Arkansas Water and Air Pollution Control Act and the regulations promulgated thereunder. This inspection revealed the following violation: Wastewater was surfacing in areas of the septic system. This is in violation of Condition 1 of the permit. The above item requires your immediate attention. Please submit a written response to this finding to Cindy Garner, Water Division Enforcement Branch Manager, of this Department. This response should be mailed to the address below. This response should contain documentation describing the course of action taken to correct each item noted. This corrective action should be completed as soon as possible, and the written response with all necessary documentation (i.e. photographs) is due by May 23, 2009. There is a distubution valve past the 1250 combo tank that can be turned and feeds to two-2000 gallon tanks if more storage is needed due to groundwater. 6A. SUMMARY OF FINDINGS/COMMENTS Areas observed where parts of the system appeared to be surfacing. On May 11, 2009, I spoke with Dr. Hokett. Dr. Hokett stated that landscapers were adding additional top soil to the line. Dr. Hokett stated that the d-box was full of groundwater. June 17, 2009 Laura Hokett DVM, Best Friends Animal Hospital and Pet Hotel 4175 East Mission Blvd. Fayetteville, AR 72703-4388 RE: Best Friends Animal Hospital, Permit No. 4864-W, AFIN 72-01633, Response to Inspection Dear Dr. Hokett: The Department has received your response to the May 6, 2009 inspection of your facility by our District Field Inspector, Allison West. Your letter appears to adequately address the discrepancies identified during the visit. The Department assumes the corrective actions taken will be maintained to ensure consistent compliance with the requirements of the permit. Acceptance of this response by the Department does not preclude any future enforcement action deemed necessary at this site or any other site. The Department will keep the inspection and response on file. If future violations occur that require enforcement action, the Department will consider the inspection and response as required by the Pollution Control and Ecology Commission Regulation No. 7, Civil Penalties. This regulation requires the Department to consider the past history of your company and how expeditiously the violations were addressed in determining any civil penalty that may be necessary for any future violations.
