2015 Application Form Scholarships for Graduating High School Seniors Please indicate individual scholarship(s) you wish to apply for by marking with an ‘X’; Mark only those for which you are qualified! 2015 Application Form Scholarships for Graduating High School Seniors

Please indicate individual scholarship(s) you wish to apply for by marking with an ‘X’; Mark only those for which you are qualified! (Read the criteria carefully.) All applicants must be

graduating from a Benton County High School unless otherwise specified.

The following scholarships are based on financial need. Be sure to submit the correct paperwork as listed on the application. All other scholarships are merit-based and financial need will not be considered.

____The Herb & Peggy Ballard Memorial Scholarship: one $600 award for payment of tuition and books at any accredited institution of higher education. Student must demonstrate financial need; GPA 2.5 or above; open to graduating seniors of Monroe High School only.

____The Citizens Bank Scholarship: one $700 award based on financial need, academic performance and leadership. Must be a dependent of an employee of Citizens Bank to apply; can reside in any Oregon county.

____The Loren J. & Audrey W. Smith Scholarship: one $1,800 award for study at Linn Benton Community College. Student must demonstrate financial need. ____The Ruth & Julian Adams Memorial Scholarship: one $1,700 award for the study of journalism at any accredited institution of higher education.

____The David Bruce Belcher Scholarship: two $1,000 awards to study nursing, therapy or special education. Must have a desire to help persons with developmental disabilities.

____The Dr. Benjamin A. & Elizabeth McFeron Cathey Memorial Scholarship and the Peter & Barbara Hutton Scholarship: one $1,800 award for the study of medicine or health care.

____The John & Nancy Dennis Scholarship: one $1,200 award for any course of study at Oregon State University.

____The Robin V. Jensen Scholarship: one $2,000 award to pursue a bachelor degree at an accredited university with an emphasis on Forensic Science or Criminal Justice. Must be active in sports and community service throughout high school and have a GPA of 3.0-3.5. Open to graduating seniors of any high school in Benton County, but preference given to Crescent Valley high school applicants. This scholarship is renewable.

____The Knights of Pythias Scholarship: three $2,000 awards for pursuit of vocational training rather than university studies. This scholarship is renewable.

____The Otto J. & Adelia Hahn Scholarship: four $2,000 awards for the study of mechanical, nursing or manual arts at Linn Benton Community College. Preference will be given to applicants living in rural areas. This scholarship is renewable.

____The Allen & Jean Peters Family Scholarship: a minimum of $1,000 award for the study of a health sciences related major at Oregon State University. Applicant must have resided in Benton County for at least three years.

____The Dale & Thomasina Trout Scholarship will grant seven $2,000 awards for post high-school study. Applicant must be in the upper half of the class based on academic performance and have a proven record of activities or athletics. This scholarship is renewable. Preference is given to students not likely to receive other scholarships. _______________________________________________________________________________________The following scholarships include high school restrictions:

____CHS Only: The Robert W. & Cleo Wakefield Burtner Memorial Scholarship and the Maybelle Game Carlson Scholarship: one $740 award for the study of history, environment, journalism, home economics or religion.

____CHS Only: The Emily & Vic Logan Tuition Scholarship: one $1,400 award for the study of history, foreign affairs, or the political process. Must have a GPA of 3.3 or higher.

____CHS or CVHS: The Coopey Boys' Memorial Scholarship: one $1,800 award to a student with a physical handicap or a serious chronic health problem.

____CHS or CVHS: The John C. & Jeanne L Mackey Endowed Scholarship: one $2,000 for study at any of the public universities in Oregon. Applicant must be US citizen, have a GPA of 3.0 or higher, and a proven record of activities and/or athletics.

____ CHS or CVHS: The Marjorie Rowett Sutherland Scholarship: one $600 award for post high-school study. Applicant must have a minimum GPA of 3.0 and be pursuing a business degree. Preference given to those students who excel in high school curriculum related to business.

____PHS only: The Harry & Lorraine Lagestee Scholarship: two $2,500 awards. GPA must be between 2.5 and 3.5. Recipient must have participated in at least two sports during school. Please include the number of years you’ve attended PHS. ________

____Monroe HS only: The Herb & Peggy Ballard Memorial Scholarship: see description listed on page 1 of this application

For those scholarships that are based on financial need, include only the first page of the Student Aid Report (SAR) as generated from submission of the Federal Application for Federal Student Aid (FAFSA). This first page includes the Expected Family Contribution (EFC), which is how we will determine financial need. For more information: https://fafsa.ed.gov/ If it is not possible for you to submit this report then please ask your high school counselor to call the foundation office on your behalf.

Submission Process:We require electronic submission of the application, preferably in one file. Please email the completed application to [email protected]. You will receive an emailed confirmation of receipt. Follow-up if you do not. If you have completed the form by hand, you’ll need to scan the completed application to email in the files. If you do not have scanning capabilities, you may take the signed submission packet to any number of copy and print shops to have it scanned.

Applications are due to Benton Community Foundation by Friday, March 27, 2015.
