Why BENEFITSPLUS Enrollment?

Under the Affordable Healthcare Act, the healthcare options available to many individuals are changing.

To assist our employees in understanding the healthcare benefits that we offer so that they can make informed decisions, between July 16th and August 29th, Fresenius Medical Care North America (FMCNA) will be hosting informational sessions to help all benefits-eligible employees better understand the company’s existing health plan options as well as new voluntary benefits offerings.

BENEFITSPLUS Enrollment Goals

• Educate employees on the company’s existing health plan options and helping to choose the plan that is the bestfit for them and their families.

• Introduce the New Voluntary Benefits offerings; Accident Insurance, Critical Illness Insurance and Universal Life Insurance

• Make the enrollment process a positive experience for all employees

Who is eligible?

• All benefits-eligible employees scheduled to work at least 30 hours per week.

• Eligible employees may also enroll their legal spouse, domestic partner* and dependent children.• Dependent Children – age limits vary by plan

• May be the natural child, stepchild, legally adopted child, child placed for adoption or other child for whom the employee has permanent legal custody.

*Appropriate proof of domestic partnership must be submitted to Fresenius Medical Care.

What is Best Fit Plan Advisor?

The bestfit Plan Advisor helps the employee evaluate their current medical plan, given the employee’s health status and that of their covered family members.

• The tool estimates the amount of health care the employee is likely to experience in a given year and the out-of-pocket expenses the employee is likely to incur for each plan.

• Using the Best Fit tool with a Benefits Educator will help the employee understand the relationship between their healthcare consumption and their out-of-pocket costs.

Group Accident Insurance Accident insurance provided by Unum can pay benefits based on the participant’s injury and the treatment received, including emergency-room care and related surgery. The benefit can help offset the out-of-pocket expenses that medical insurance does not pay, including deductibles and copays. • On & Off the job coverage• Sickness Hospital Confinement Benefit*- pay employee, spouse or child a daily benefit if hospitalized for a

covered sickness. $100/day, to a maximum of 30 days, for employees and spouse; $75/day for children.• Family coverage options are available**:

– employee & spouse or domestic partner– employee & dependent children– employee, spouse or domestic partner & dependent children

Injuries include: Expenses include:- broken bones - emergency room treatment- burns - outpatient surgery facility- torn ligaments - doctor office visit- cuts repaired by stitches - hospitalization- coma due to a covered injury - occupational therapy- eye injuries - speech therapy- ruptured discs - chiropractic visit- concussion - physical therapy

Group Critical Illness Insurance Specified critical illness insurance offers a lump sum benefit payment, upon initial diagnosis of a covered critical illness, that can be used however the participant choose. The participant can choose a lump sum benefit up to $50,000 that’s paid directly to him/her at the first diagnosis of a covered condition. This coverage can be used more than once. If the participant receives a full benefit payout for a covered illness, his/her coverage can be continued for the remaining covered conditions.

• Employee Coverage - $5,000 - $50,000 in $1,000 increments – Guaranteed Issue up to $30,000• Spouse Coverage* - $5,000 - $30,000 in $1,000 increments – Guaranteed Issue up to $15,000• Child(ren) Coverage **- Automatically covered at 25% of employee benefit amount (up to age 26)

• Wellness Benefit - $50 per insured / calendar year if a covered health screening test is performed. • Multiple Pay Outs - Each condition is payable once per lifetime. Occurrences must be separated by 90 days. • Pre-Ex – N/A• Benefit Waiting Period – N/A• Additional Covered Conditions for Dependent Children: - Cerebral Palsy, Cleft lip or Palate, Cystic Fibrosis, Down

Syndrome, Spina Bifida

*Spouses issue ages are 17 through 64. **Children issue age newborn up to 26 th birthday.

Universal Life Insurance

Why Universal Life Insurance? • Helps provide permanent financial protection• Is a financial tool that helps you manage life at every stage – from supporting a family to sending

your children to college• Builds cash value over time that you can access for life’s challenges and life’s opportunities

Family Coverage is available – an employee may protect their spouse, children up to age 23 and grandchildren up to age 18.

Guaranteed Issue* – no health questions!• Employee is up to $150,000• Spouse is the lesser of the face amount purchased by $3.00/week or $5,000• Children is the face amount purchased by $4,00/week

*An employee must apply for coverage on themselves for the dependent to be eligible for Guaranteed Issue.

• Reserve a semi-private location for the enrollment that is centrally located and familiar to employees at your location. Please remember that employees will be discussing their personal information.

• Suggested locations: extra office, corner of the break room, etc.

• Needs to be equipped with two power outlets, a table and 2 chairs (the Benefit Educator will bring all equipment and materials)
