Obesity / Healthy Living Update:
The Clay County Girls Scouts held a one-day educational session to promote
the importance of healthy living through proper nutrition and physical
activity. The event was held in May 2012 with 28 girls in attendance.
Continue to celebrate successes
throughout the action cycle.

Access to Healthcare:
The hospital is in the process of transitioning their electronic healthrecords system to support patient tracking as part of the medical
home model.
Discussion / negotiations are underway for identifying additionalspecialists.
Cancer:
The hospital promoted special Mothers Day pricing for womenshealth screenings.
Contact subcommittee leaders every
23 months for updates.
Branding G. Paule The IPLAN Committee divided into 3 groups to participate in brainstorming
activities related to brand development. The goal of the branding activity
was to generate ideas that could be used to rebrand our IPLAN efforts. We
will use the newly developed brand as part of our marketing strategy to
promote our health improvement initiatives to the community.
Committee members will cast their
votes for the new name, tag line, and
logo via SurveyMonkey. The results
will be announced via email.

Each group was given a copy of the mission, vision, and value statements; a
list of ideas created by the Branding Subcommittee; and a picture that
reflects the community (family, kids playing sports, and agriculture). Using
these three items, the groups were asked to create a name and tag line that
could be used to brand our IPLAN. Each group presented their ideas to the
entire Committee.
