SITE HUMAN PERFORMANCE EVENT-FREE DAY RESET CRITERIA Definition of a Site Human Performance Event-Free Day (EFD)

Reset – When an event, as described in the table below, occurs

as a result of either of the following:

• an initiating action (error) by an individual or group of individuals (Re)

(event resulting from an active error)

• an initiating action (not an error) by an individual or group of

individuals during an activity conducted as planned (Md) (event

resulting from a flawed defense or latent organizational

weakness created within the last 18 months)
