A. JOB PACKAGE SCOPE

1. GENERAL SCOPE OF WORK

a. This Job Package covers the excavation support by ReynoldsElectrical & Engineering Co., Inc. (REECo) and Raytheon ServicesNevada (RSN) to the U.S. Geological Survey (USGS) associated withthe deepening of existing Trench 14. Trench 14 is located on thewest side of Exile Hill about 2 kilometers east of the ConceptualPerimeter Drift Boundary. It was excavated across the Bow Ridgefault in the early 1980s. Maps of the present exposure arecurrently being finalized by the USGS and an extensive sampling andanalysis program by the USGS and Los Alamos National Laboratory (LosAlamos) is near completion. In the trench, the fault zone in theMiocene Paintbrush Tuff and Quaternary colluvial deposits has beenfilled with vein-like deposits of calcite and silica that extend tothe floor of the trench. In order to finalize conclusions regardingthe origin of the calcite/silica deposits, further evaluation of thedepth and character of these deposits is necessary. This will beaccomplished by deepening the existing trench to a total depth of asmuch as 30 feet. The present excavation is 10 to 12 feet deep,about 16 feet wide, and about 100 feet long. The deliverableidentified for this Job Package is the Excavation Completion Reportin which REECo, RSN, the Principal Investigator (PI), and YuccaMountain Site Characterization Project Office (YMPO) document thatthe excavation and shoring, if required, is complete and follow-onmapping and sampling activities can begin. It is anticipated thattrench excavation will begin on July 8, 1991, and continue for asmuch as two weeks.

Environmental compliance approval for this work was denied by theOperations Control Branch of the YMPO because the required permitshad not been granted by the State of Nevada, although preactivitysurveys have been completed. Excavation should not begin until theSite Manager is informed by the Director, POCD that the requiredpermits have been received.

In addition, a biological resources preactivity resurvey isrequired five days or less before excavation begins.

e. SIGNIFICANT INTERFACES

o The Field Test Coordinator or designee will monitor alltechnical work at the site.

o REECo will be responsible for safety coordination duringexcavation.

o RSN will provide a soils engineer at the site during excavation.

o The YMSO Safety Specialist will make a final inspection prior tocompleting the Excavation Completion Report.

o The soils engineer, safety specialist, and the PI will jointlydetermine the final excavation dimensions and, if appropriate,shoring requirements.

2. SPECIAL FIELD CONDITIONS

A. SAFETY TRAINING REQUIREMENTS

No unusual hazards have been identified in the work scope coveredby this job package. Occupational Safety and Health Administrationstandards relating to such work provide adequate coverage and nospecial training is deemed necessary.

The general "8 hour" field safety training course must be completedby all personnel performing field work under this job package.Personnel wishing to visit the job site must be escorted by anindividual who has completed the training.

b. FIELD ORIENTATION REQUIREMENTS

The General Employee Training (GET) field orientation class must becompleted by all personnel performing work and non-escortedpersonnel visiting the job site.

c. RESOURCE ALLOCATIONS

Light duty vehicles and portable radios are available at the SiteOffice through the Logistics Coordinator. Requests for temporaryand permanent office space may be made to the Site Manager or theTechnical & Management Support Services (T&MSS) FOC Manger.

All field personnel must be aware of the Field OperatingInstructions (FOI) system and must comply with applicable FOIs forthose activities performed.

Head counts and location of personnel in the field must be reportedto Operations each day before 08:30 hours. each project participantis responsible for reporting Its own head counts unless otherarrangements are mnde with Operations in advance.

Daily progress reports must be provided to Operations by 15:30 hourseach working day. Operations staff will make the appropriatedistribution.

9. INTERFACES

Field personnel should be aware that non-project personnel are alsoengaged in activities in Area 25 that could interrupt or evenconflict with the performance of the job package work scope. Someof these non-project personnel may be Involved in special activitiesfor the Department of Defense. Conflicts with these personnel areto be avoided (including work interruption if necessary) and theSite Manager should be notified of such interactions orinterruptions.

h. ADMINISTRATIVE HOLD POINTS

Surface disturbing work shall not be performed until documentednotification is given by the Site Manager to REECo stating that thenecessary permits have been received and such work may commence.
