New to Aetna? Your doctor is probably already in the Aetna networkWith over a million doctors, hospitals, labs and other health care providers in the Aetna network, chances are yours is one of them. You can find out right now by visiting DocFind® at www.aetna.com/united. A network comparison showed that 95 percent of the providers currently used by United co-workers are in the Aetna network. That means that the majority of United co-workers will not need to select a new provider to receive the highest level of coverage.

SmartCare™ Seeing a top quality SmartCare provider:

•Could improve chances of survival from an acute or chronic medical condition like a heart attack, coronary artery bypass, stroke and many others.

Map out your wellness plan Beginning January 1, 2014, you may link to the Simple Steps Health Assessment through Aetna Navigator. It takes less than 20 minutes to answer questions about your lifestyle, health history and other factors related to your overall wellness. Then follow the steps to wellness.

•Get a personal health report — When finished, you’ll get a health report that you can share with your doctor and discuss any concerns.

•Get a personalized action plan — You’ll also get an action plan suggesting ways for you to get healthier and the Aetna programs that may be a good fit in getting there.

•Start getting healthy — Follow your action plan and check out any programs recommended for you. Many programs can be completed online at your own pace.

Use the CarePass® tool to guide you to wellness Everyone has their own journey to becoming healthy. CarePass is another option that can lead you to wellness. Think of it as your personal guide along the way. Challenge yourself by virtually running from one city to another. Lose weight to get into those beloved jeans. Or create your own goal. You will have access to FitBit, Lose It!, Body Media® and Zipongo — and more!

iTriage gives you quick, convenient access to information about symptoms and conditions, connects you to nearby doctors in your insurance plan network, and helps you save and manage your health and medical information. Check ER wait times, request an appointment and even check in prior to arrival at select facilities.

Try a virtual doctor appointment The Teladoc® service puts you in touch with a doctor no matter where you are.* Just provide your health history information. Then, when you need care, request a consultation. It’s a convenient way to get care when you’re away or not feeling well enough to leave your home — for example, if you have a fever, upset stomach or headache — or a sick child. A board-certified licensed doctor will review your case and talk with you by phone.** Online video consultations are also available in some areas.*** The doctor can advise you on what to do or prescribe a medication for you. Your cost for the service is the same as your plan costs for your doctors’ visits.

If any other covered family members have health risks, they may choose to work with the same nurse.

Let a health coach make lifestyle changes easy Sometimes the best way to meet your health-related goals is with some personal, focused attention. With Healthy Lifestyle Coaching, you work one-on-one — on your schedule — with a highly trained wellness coach to help improve the way you feel every day. It’s a powerful step available at no extra cost to you. The program offers personal coaching and support from experts who specialize in areas like tobacco cessation, weight management, nutrition counseling and more.

When you sign up, we’ll pair you with a coach who’ll work with you throughout the program. Your wellness coach is your ally who will help you make the changes you want to make and celebrate your successes with you, too! Take it as fast or as slow as you want. You decide how and when you work with your coach.

Make the right decision when it comes to surgerySurgery is often an extreme approach to care for most health conditions, and in many cases, there are other solutions. The Welvie Surgery Decision Support Program can help those who are considering surgery make the best choices for their unique situations.

The online program includes easy-to-understand videos that guide you through six steps.
