P lanning for your death is not a pleasant thought, and as such many of us choose not to do

so. The young collegiate, new dad or married couple purchasing their first home are not worrying about their final wishes because they have bigger things going on in their lives right

now. According to Dr. Joan Harrold, Vice President and Medical Director of Hospice & Community Care, there is no larger issue or better time to think about the normally tiptoed-around advance care planning. “Everyone needs to have an advance directive. I believe this so strongly that both of my children have one.” Anyone over the age of 18 can write their wishes in the form of an advance directive.

What is an advance directive, and what does it say, exactly? Dr. Harrold explains it simply as laying out what you want at end of life, and most importantly, naming who you would

like to speak for you and make medical decisions in the event that you are not able to do so. “Many think this document has to explain in detail what you want in all medical aspects, but it doesn’t have to be detailed, or it could be. The most important part

is putting in writing your medical power of attorney,” she says. This decision is very personal and starts the conversation with whomever you would choose to ensure that they know your wishes and desires in the event that they were called

upon to make those difficult decisions. This is not just for the elderly, but also young adults and those in their 40s who may be very healthy. Dr. Harrold provided an example: A fairly young person is in an automobile accident and

cannot make medical decisions. Without an advance directive who is in charge of gathering information, deciding if/when to have surgery, obtaining other medical options, officially signing off on any medical procedures? With an involved family there can be multiple opinions, or without there may be no one. If there is an individual named, the big decisions have already been discussed with the patient and they will know how to proceed.

The point of an advance directive is to keep the patient in charge of their

healthcare, even when they cannot speak for themselves. Without a plan, the law has a hierarchy of those who can make decisions and puts it in order of healthcare preferences. This takes the control out of the patient’s hands, and even certain family members or close partners and friends. Dr. Harrold says, “I had a patient without an advance directive and their family excluded their long-term partner from any decisions in how that patient lived their last days, even though the partner would’ve known better than any of them.”

When choosing your medical power of attorney Dr. Harrold points out that it is not a measure of how much you love someone, nor how much they love you. Choose someone who will honor your wishes completely, even if they may not understand them. Explain what guides you in life and what is important. “I had a very successful advance directive case where a patient named her ex-granddaughter-in-law as her medical power of attorney. Her wishes were followed,” Dr. Harrold said.

If you have a specific disease, such as lung disease or ALS, your advance directive will be more specific than if you are a healthy young person. However, both must start with a conversation about what is important to you and naming a medical power of attorney. The conversation can seem intrusive, but it is a crucial part of a simple preparation. Dr. Harrold says, “We all have wishes and personal beliefs that drive us every day and they should carry through to the very end.”

Making Your Wishes Known

National Healthcare Decisions Day is April 16

M any of us probably haven’t given too much thought as to what we want our care to look like when we reach end of life. It can be an uncomfortable topic

to consider and discuss. Nonetheless, advance care planning is extremely important to your healthcare plan. That’s why attorney Nathan Kottkamp founded National Healthcare Decisions Day, or NHDD, in 2008. In his words, “both potential patients and healthcare providers can do a much better job of making our wishes known and then honoring those wishes.”

NHDD is all about facilitating conversations with your loved ones to make a plan and define wishes for your care and theirs at end of life. On April 16, churches, public venues, and healthcare providers, including Hospice & Community Care, will be celebrating this day by hosting events that aim to raise awareness of the importance of advance care planning and provide resources for making informed decisions.

You can learn more about advance directives by coming to one of the information sessions on April 12 or April 14. Hospice & Community Care is partnering with Lancaster General Health to offer this presentation and question and answer panel. Moderated by Maggie Costella, Vice President

Taking care of so many patients with varied needs at end of life, medical conditions and home

environments is a challenging task, but one that can be done with the help of many. Hospice & Community Care uses a team approach to provide the most comprehensive level of care to patients. The Blue Team shared during a recent team meeting the importance of collaboration, what it means to work together and how this makes such a significant difference.

The Blue Team is led by RN and Team Leader, Elaine Muschlitz, a 20-year Hospice employee and includes a chaplain, two social workers, six registered nurses (RN), four licensed practical nurses (LPN) and four hospice aides (HA). These 18 staff take care of 50-70 patients on any given day. Each team is given a specific geographic area in which their patients live; the Blue Team’s boundaries are up the 501 corridor to Myerstown, east to Reading and south on 222 to Lancaster. They see patients in Lancaster, Berks and Lebanon counties. The team members describe patients as having a variety of needs, all of which cannot be fulfilled by one person. They are in constant contact with one another when on the road or in a patient’s home, making sure needs are met. A phone call provides the most immediate response if timing is crucial, but text messages and emails are exchanged. “We are very careful because we have to rely on these forms of communication, but they are all secure.” Another member chimes in, “It’s nice to know at the end of the day you aren’t alone and can ask for advice about a patient, or update notes and receive a response from

several other team members within minutes. There is always someone there.”

The support each team member gives to one another is encouraging, and can be seen during the team’s weekly meeting. “We respect each other and our expertise. We take advantage of it because we understand that we cannot do it alone,” says Libby Kimber, Social Worker. For example, an RN can’t address a patient’s spiritual needs and will call on the chaplain, while the aide may notice a patient’s labored breathing while bathing and call the RN for a medication check. The team supports their roles and boundaries, but will step in to help whenever they can. Nilda Roman, a Hospice Chaplain recalls a time when she was walking in the door to a family’s house whose parent had just passed and she received an alert that another patient passed. “I couldn’t be in two places at once, so I called the LPN, and she told me she knew I was with the other family and she had already contacted the clergy so I could take my time. It was a relief knowing that I could give my full attention to the family at that moment and know the other family was being taken care of.” An RN shared a time

she was down to her last catheter and desperately needed another for her patient. So she sent a text to the team and received a response within minutes that another RN was close by and would bring her the equipment that she needed. ‘This allowed me to provide the care that my patient needed without delay.’ The team notes that families notice how well the team works together, and they can tell. “I love when we get to go on joint visits because we get to see each other in action,” a team member says. “And that is noticeable to the family; how we interact together and with them.”

It all goes back to the patients and families and providing the best care possible. The team approach allows consistency in who is providing care. The closeness of the team allows anyone to instantly update the rest of

the team when there is a significant change, such as a symptom, increased or new pain, or when at end of life. Each member has their role, very unique to the team.

Hospice aides provide personal care such as bathing and dressing, and become closer to the patient. The aides are, essentially, ‘the eyes and ears for the LPN and RN’ as one team member described. They build a strong sense of trust with the patient and family and ensure the LPN and RN’s strategies are working.

LPNs are a bridge between Aides and RNs. “We can come in and do the personal care activities, but can also fulfill some of the RN’s duties to make sure all of the medications are working and the patient is comfortable.” LPNs offer updates to RNs and can provide a clinical outlook to the family.

RNs offer skilled care and the technical side of hospice. They work to control a patient’s pain and symptoms and are contacted when a patient’s condition changes. RNs create a care plan to maintain and control a patient’s comfort through medications and work with the LPNs and Aides to ensure the care plan is being followed. The RN documents what is needed to fulfill Medicare and insurance requirements.

Social workers have a relationship with each discipline on the team and are called upon to provide outside support and resources to the patient and family. They may help the patient get enrolled in Meals on Wheels or provide the

family with instruction on how to help their loved one in and out of bed. They are also there to listen without judgment.

The chaplain offers spiritual guidance for patients and families, and can also serve in that capacity for the team. Nilda describes her role as being with the family during this difficult time. “Many patients have questions, fears, or even regrets as they near the end of life, and I feel honored that I can offer them reassurance.” She gives hope to the family and allows them to grieve.

The team leader is known as the ‘keeper of the flock’, providing direction to each staff member and keeping everyone together. The collaboration on each of the 50–70 patients is consistent

and quality-driven. Elaine Muschlitz reminds everyone that what they do each day matters and is a sounding board when issues arise as well as a direct line to other resources.

In the end, the Blue Team all agree on some basic points: the patients and families come first; humor helps them in almost every situation; they share a sense of pride in what they do, and walking the journey with patients is monumentally valuable to each of them in their roles.

Hospice & Community Care has many people who dedicate their time and resources in a

multitude of ways to the organization. Lorraine Oetting is one such individual who is involved in many facets of Hospice & Community Care. She contributes much of her volunteer time to patients and supporting efforts to raise funds for Hospice.

Lorraine was born in Binghamton, NY, where she graduated from high school and laughs as she recounts the twisting road to meeting her husband. She was babysitting on New Year’s Eve and received a random phone call from three young fellows. Intrigued, she held a conversation with them for hours and they came to visit her the following day. One left to join the Navy, where he met her husband, Bill, who was looking for a pen pal. “I still have his first letter from so long ago,” Lorraine remembers. They married and moved to Brooklyn; an exciting place for Lorraine who grew up in the suburbs. After Bill received an engineering degree, they made a move to New Jersey and had two daughters.

“I was very involved in politics, the girls’ PTA and the community, and spent my time working as a bookkeeper. Bill supported me in all of it,” Lorraine said. “I always wanted to go back to college, so when the children were grown I enrolled at Rutgers.” Ironically, she attended college at the same time as her daughter, graduating with a degree in accounting. She became the controller at a pharmaceutical company where she remained for years. Bill and

Lorraine retired on the exact same day and moved to Easton, Maryland.

After their move, her step-father became ill and the local hospice was called to care for him. He was still living in New York, so effective communication between Lorraine and the hospice provider was essential to his care. ‘They were great and kept in touch with me constantly until he passed.’ Her experience with the hospice in New York prompted Lorraine and Bill to begin volunteering at the local hospice in Talbot County, MD. Bill would cook dinners for the patients in its inpatient center, while she volunteered to care for the patients, and occasionally make breakfast. She notes that it was always an upbeat atmosphere and she enjoyed being around the patients and families.

They moved to Lancaster in 2005 and first volunteered with Hospice & Community Care to raise funds for Camp Chimaqua. Lorraine and Bill also attended the Labor Day Auction and purchased a cruise donated by long-time Hospice supporter, Thelma Flory. “We had wonderful dinners with Thelma and heard her story,” Lorraine explains. “We decided to become more involved so we began selling raffle tickets.” Sadly, Bill passed away unexpectedly in 2010. Lorraine turned to the Pathways Center for Grief & Loss to provide support while she grieved the loss of her husband of 56 years.

Years later Lorraine was again involved with Hospice & Community Care, but not just selling raffle tickets. A close friend became ill and received hospice care. After he passed she found the

Pathways Center for Grief & Loss again beneficial. “The support groups at Pathways helped me a lot. I went to my church but it wasn’t quite the fit I was looking for, and Pathways was.”

In 2014 she joined the Women’s Giving Circle, a group of women who each contribute $500 annually and pool their gifts to fund Hospice programs and projects. She likes to know where her money is going, and to have a say in how it is being used. She also completed volunteer training last year. “I’m looking forward to being placed with patients and working in nursing homes. I like having that interaction.” She is also a regular financial supporter of the organization. She describes it as a wonderful feeling, whether you are donating your money or your time.

Being an accountant, Lorraine is very knowledgeable about her finances, taxes and estate, as well as how certain decisions will affect her portfolio. Many of us let ‘the professionals’ make our financial decisions, but not Lorraine. These decisions include all of her charitable contributions, and a new law as to how individuals 70 ½ or older can take their IRA distribution makes her very happy.

Lorraine breaks the direct IRA charitable distribution down so it is much easier to understand:

Anyone who is 70 ½ or older and has a traditional IRA must take a distribution each year, or be penalized. So, if you fall into this category and you don’t take it, you’re losing money. You have the option to direct your distribution to a charity, without taxes, thus decreasing your adjusted gross income (AGI) and, in

turn, your overall taxes. However, this law was voted on each year and was usually adopted in December. Most people wanted to have their IRA distributions in place before the end of the year, and thus most did not take advantage of this benefit to both charities and themselves.

In 2015, Congress approved a permanent exemption for the charitable distribution so now no one has to wait until December to make their contributions to charitable organizations and individuals can plan in advance. Lorraine stressed that this allows those who have to take an IRA distribution can do so by way of a direct gift to charity, without waiting. “My thought is, I have to take it but maybe I don’t need it, and giving to charity is a wonderful option. It helps them and it helps me. I choose to give my charitable IRA distribution directly to Hospice & Community Care and I feel very good doing it,” Lorraine explains.

The IRA direct charitable distribution is now permanent and she notes the most important points:

• It will satisfy part or all of your required distribution.

• You can donate up to $100,000 tax-free.

• It lowers your AGI.

Most importantly is the benefit to charitable organizations.

Lorraine is a tremendous supporter of Hospice & Community Care, and a wonderful addition to the Women’s Giving Circle. She simply says, “I am very grateful to God for all I’ve been able to do.”

She was the volunteer for my mom when she was under Hospice care. Erma came in and

gave me the chance to run errands or just go out to lunch with friends. She was wonderful,” Elaine Kreider said. She is talking about her decision to volunteer with Hospice & Community Care. The connection she made with her mother’s Hospice volunteer, Erma, turned into a treasured friendship that has continued, and that impact as well as the care her mother received inspired her to give back and become a volunteer herself.

Elaine grew up in Lancaster on a farm and graduated from Penn Manor High School. She married her high school sweetheart, but tragedy struck and her husband passed away after only six months of marriage. She gave birth to his son shortly after his death, and six years later she married her current husband, with whom she just celebrated her 46th anniversary. Elaine retired from Armstrong after 37 years and worked at her church’s preschool for five years. In her spare time she enjoys knitting and cross-stitch, working outside and traveling with her husband.

Looking back to her initial experience with Hospice & Community Care, Elaine’s mother was ill and discharged from the hospital under Hospice care in 2011 and lived with her and her

husband for almost a year. “I was the primary caregiver, and I don’t know what I would have done without Hospice,” she said. “They were so caring and compassionate. I truly could not have done it alone.” After her mother spent seven months receiving hospice care at home with Erma regularly providing respite, Elaine’s mother was transferred to a nursing home where she died a few months later. Elaine wanted to give back after her experience with Hospice. She was not new to volunteering; she had been volunteering at the Blind Association and her church for years.

To say Elaine has a full schedule of volunteering is putting it lightly. Elaine is a determined volunteer and lists her duties, “I volunteer in Home Hospice once a week. Every Friday I file paperwork on Good Drive, twice a month I deliver mail, and every other Monday I do laundry at the Inpatient Center.” When asked what she enjoys the most it only takes a moment for

her to respond with ‘Home Hospice.’ She had that in mind when she attended the volunteer training, but she enjoys all of the work she does with Hospice

& Community Care. “Working with Home Hospice is a different experience. I get to interact with the patients and meet their families. It makes you feel so good to know that

you’re giving them a break, even for a short time.”

For anyone who is thinking about joining the ranks of volunteers who make a difference every day at Hospice, Elaine says just go for it, it’s so worthwhile. “Before I was involved I was intimidated but there are so many things you can be involved in and ways you can help.” Her help in so many capacities is appreciated greatly by Hospice & Community Care and by the countless patients and families whose lives have been impacted by the time she has spent with them. President’s LetterThe past year has given us many triumphs as an

we began offering palliative medicine services at WellSpan York Hospital; and most importantly we supported nearly 9,000 bereaved adults, teens, and kids and provided care to a record 3,000 hospice patients and families. Hospice & Community Care maintained the highest quality of care for every patient and family and this standard of exemplary care was reflected in our Joint Commission accreditation. I feel fortunate to work alongside the dedicated staff who have made all of these incredible accomplishments come to fruition, the countless volunteers who contributed their time and efforts, and the community that has supported us throughout the year. These successes all lead to one end: more people receiving the best end-of-life care and support possible.

Now that 2016 has begun, we anticipate new opportunities to extend our mission. As of January 1 we are offering a program that no longer forces patients to choose between hospice support and curative treatments. Supportive Care is an innovative program that allows us to reach more patients and provide support without having to make a choice. Our Palliative Medicine Consultants group continues to grow and will offer more services in York this year in conjunction with WellSpan Health. We hired many new staff to continue providing the most comprehensive hospice care as we serve an increased number of patients and families. I am very excited for what this year has to bring, our 36th year as the leading provider of hospice care and bereavement support in the area. It is only through our dedicated staff, community support, and our committed, incredible volunteers that we are able to continually grow and live our mission.
