If you enroll in a new plan or change plans for 2018, you will receive a member ID card in the mail about two weeks after enrolling. If you will be seeing a new doctor, it’s a good idea to schedule a first visit once your coverage is effective January 1, 2018.

If you live in Massachusetts, Maine, Rhode Island, Connecticut or New Hampshire and are comfortable with an HMO, this plan enables you to see Harvard Pilgrim’s participating doctors, hospitals and other providers in New England.

The HMO network includes more than 68,000 doctors and other clinicians and 182 hospitals.

Do I need a primary care provider (PCP)? Yes. You must have a PCP provide your care (except in emergencies) and give you a referral for most kinds of specialty care for the plan to provide coverage for services.

Can I continue to see my current PCP? Yes, if your current PCP belongs to the Harvard Pilgrim network.

How can I find a new provider?You can use the HMO provider directory at www.harvardpilgrim.org/mfs or call Harvard Pilgrim at the number on the first page of this brochure.

Do I need referrals to visit specialists? Yes, you need your PCP’s referrals for most visits to specialists. Referrals are not necessary for some services, such as routine eye exams or most gynecological care.

This flexible plan lets you go outside of Harvard Pilgrim’s provider network and doesn’t require referrals to specialists.The Access America network offers access to 5,700 hospitals and care from 960,000 doctors and allied health professionals across the United States that make up UnitedHealthcare’s extensive network.

Do I need a primary care provider (PCP)? No, although we encourage you to have one to help coordinate your care.

How does in-network coverage work?You have in-network coverage when you receive care for covered services from providers and hospitals that participate in Harvard Pilgrim’s regional network, and UnitedHealthcare’s network across the country. Your out-of-pocket costs are lower when you visit an in-network provider.

How does out-of-network coverage work?You have out-of-network coverage when you receive covered services from providers and hospitals that do not belong to the network. Typically, you will pay more for out-of-network coverage, and non-participating providers may bill you for the differences between their charges and the amount Harvard Pilgrim pays for covered services.

Do I need referrals to visit specialists? No. Just keep in mind that when you’re going to be admitted to the hospital, services are covered according to which providers you use. For example, if a non-participating doctor performs your surgery at a participating hospital, you have out-of-network coverage for the doctor’s services and in-network coverage for the hospital’s services. Except in an emergency, you must notify Harvard Pilgrim in advance of a hospital admission when non-participating providers and hospitals are involved.

This flexible plan may be a great option for employees who want more control over their health care spending. Compared to the Best Buy plan, the Saver Plan offers lower monthly premiums, but with a higher deductible for most covered services. To help offset the costs of qualified medical expenses*, this plan allows you to set up a Health Savings Account (HSA) through pre-tax payroll deductions. As your employer, MFS will also contribute to your HSA—an annual contribution of $750 individual / $1,500 family. (The combined contribution from you and MFS cannot exceed the annual contribution limit set by the IRS.)

The Health Saver plan works similar to the Best Buy plan—you’re not required to have a PCP or get referrals to specialists. You have access to 5,700 hospitals and care from 960,000 doctors and allied health professionals through UnitedHealthcare’s extensive nationwide network. Your out-of-pocket costs are lower when you visit an in-network provider. When you see providers outside of the network, your costs will be higher and the provider may bill you for the differences between their charges and the amount Harvard Pilgrim pays for covered services.

You’ll benefit from tax savings: Your contributions, interest and withdrawals for qualified health care expenses are all tax-free.

Any unused amounts in your HSA can be carried over from year to year, so you don’t have to worry about forfeiting your contributions at the end of the year.

You own your HSA, so if you change jobs or retire, your money stays with you to spend on medical expenses or save for future health care needs.

Both you and your employer can contribute, and you can use funds to pay for certain health care services not covered by the HSA Saver plan, such as laser eye surgery or alternative care.

Which plan is right for you?

Live in Massachusetts, Maine, Rhode Island, Connecticut or New Hampshire and are comfortable with an HMO, a plan that enables you to see Harvard Pilgrim’s participating doctors, hospitals and other providers in New England.

The HMO network includes more than 68,000 doctors and other clinicians and 182 hospitals.

Choose this plan if you . . .

Coverage for all plansPrescription coverageAll three plans offer a 3-tier prescription drug program. When you use a participating pharmacy, you receive up to a 30-day supply. Use mail order and receive up to a 90-day supply of maintenance medications for the same copayment.

Preventive care Preventive services, such as routine check-ups, screenings and immunizations, are covered free of charge when you receive these services from an in-network provider; no member cost sharing is required. Preventive care is care you receive when you’re healthy and symptom-free. For a complete list of covered preventive services, go to www.harvardpilgrim.org/mfs.

Emergency care Harvard Pilgrim covers all medical emergencies (e.g., heart attack, stroke, shock, major blood loss, choking, severe head injury, loss of consciousness, seizures or convulsions). Just go to the nearest emergency facility or call 911 or another local emergency number. If you are hospitalized, call your PCP and Harvard Pilgrim within 48 hours, or as soon as you can (or ask someone to do this for you).

Urgent/sick careFor non-emergency care (like a sore throat, earache or flu), you should call your doctor’s office and ask for a same-day appointment. If your doctor is unavailable, you may be directed to a covering clinician or an urgent care facility. (You may be charged an ER copay at some urgent care facilities.) You may also visit a convenience care clinic, such as CVS/pharmacy MinuteClinic, for non- emergency, common illnesses; you’ll pay the same copay amount as a doctor’s office visit. Health coaches and nurse care managersNeed some extra help reaching your wellness goals? Harvard Pilgrim offers members (age 18 and older) personal health coaches—registered nurses who are certified in lifestyle management coaching. Health coaches can help you make informed decisions about lifestyle management opportunities, including blood pressure control, weight management, exercise, cholesterol management, nutrition, smoking cessation, and stress reduction and life balance. You and your coach will develop realistic and achievable goals, and your coach will stay in regular contact with you to encourage and support you. Harvard Pilgrim also has nurse care managers on staff to help with more serious health issues, such as diabetes, asthma, heart failure and more. Go to www.harvardpilgrim.org/mfs to learn more.

Want to pay a lower monthly premium, and take advantage of a Health Savings Account to pay for current medical expenses and save for future health care needs.

$150 fitness reimbursementIf you enroll in any of the MFS health plans and belong to a qualifying health and fitness facility for four months in the current calendar year, you can receive up to $150.1

Your health and fitness facility likely qualifies if it is a full-service facility that offers cardiovascular and strength-training equipment and facilities for exercising and improving physical fitness. Most “traditional” health and fitness clubs, YMCAs and Jewish Community Centers (JCCs) qualify.

Qualifying facilities also include fitness studios/facilities that offer yoga, Pilates, Zumba, aerobic/group classes, indoor cycling/spinning classes, kickboxing, CrossFit, strength training, tennis, indoor rock climbing and personal training (taught by a certified instructor).
