1. The Yokohama Season Signature Oer only applies to retail sales o new replacement tires.2. You must purchase individual sets o 4 tires (no singles or doubles) between March 15, 2012 to May

31, 2012 to be eligible or the oer.3. To receive your rebate you must ully complete this original claim orm, attach the original dated

invoice and mail to Yokohama by June 22, 2012.4. The invoice rom a Canadian retail dealer must clearly identiy the tire dealers name, invoice number,

date o purchase, product purchased (including tire size) and quantity purchased.5. Your Yokohama rebate cheque will be mailed to your address within 68 weeks upon receipt o a valid

completed claim orm. The cheque will be mailed to Canadian addresses only.6. Available at participating Yokohama Tire dealers in Canada only.7. May not be combined with any other Yokohama promotion or program.

8. Failure to provide original documents, incomplete documentation or illegible documentation will resultin the return o your claim, unpaid.

9. Please ensure that you keep a copy o all orms including your receipt or your records. We will not bereturning any proo o purchase submitted with this oer.

10. Yokohama Tire (Canada) Inc. is not responsible or lost, stolen, damaged or misdirected mail/cheques/orms11. All felds must be completed or your submission will be returned.
