Where does transition take place?

• Transition starts on the first day of enrollment• Interview student• Complete parent survey• Review discipline history• Review home school calendar and grading terms• Review graduation requirements

During placement• Meet with student to discuss needs and progress• Attend Multi-Disciplinary Team Meetings/IEP Meetings• Coordinate SAT meetings with public schools (when necessary)• Link and refer student to community resources (AA/NA, shelters, food

banks, Job Corp, HRDF, DRS, Workforce WV)• Assist with:

– Community service hours– Resume’– Job applications– ACT test registration/transportation– Assist with college application/tours/registration– Assist with the FAFSA
