    WORKPLACE SAFETY & HEALTH PERFORMANCE AWARDS

    The Workplace Safety & Health Performance Awards (WSHPA) is an annual awardpresented by the Workplace Safety and Health Council (WSHC), in collaboration with theMinistry of Manpower (MOM), to give recognition to companies or organisations that haveperformed well in safety and health through the implementation of sound safety and healthmanagement systems.

    Types of Awards

    These awards will be presented to companies, organisations and owners or management ofprojects or worksites.

    The following types of awards will be presented:

    Excellence Award (WSHPA) for companies that have achieved the Gold Awardfor 4 or more years consecutively.

    Gold Award (WSHPA) for companies with both excellent safety and healthperformance and workplace safety and health management systems. Companiesmust not have any lost time injury in 2010 to receive the Gold Award.

    Silver Award (WSHPA) for companies that have good safety and healthperformance and workplace safety and health management systems.

    Explanation of Terms used in this application

    reportable accident : any accident at the workplace that causes bodily injury to aperson resulting in

    death, the person was granted more than 3 consecutive days of sick leave

    by a registered medical practitioner for that injury, the person was admitted in a hospital for at least 24 hours for

    observation or treatment, or,

    does not include any bodily injury sustained by a person in the course ofcommuting to or from his or her workplace using a mode of transportprovided by the employer.

    employees : includes office or administrative staff and unless specified, includes

    employees of subcontractors working within the workplace;

    injury rate : number of injuries caused by accidents in the workplace per 100,000employees per year;formula is given by:

    (Number of injuries caused by accidents X 100,000) / (Average Numberof Employees for Year 2010)

    lost time injury : any loss in production time due to an accident at the workplace

    man-hours worked without any accident:

    man-hours worked covering the longest period of days without accidentbetween two accidents. When calculating the period, the date of the

    second accident must be in 2010. If the accident free period extendedinto 2011, then the second date to be used for the calculation is 31st Dec2010.

    new occupational disease: any new confirmed case of a disease specified in the ThirdSchedule of the Workplace Safety and Health Act, i.e. aniline poisoning,anthrax, arsenical poisoning, asbestosis, barotrauma, berylliumpoisoning, byssinosis, cadmium poisoning, carbamate poisoning,carbon bisulphide poisoning, chrome ulceration, chronic benzenepoisoning, compressed air illness, cyanide poisoning, epitheliomatousulceration (due to tar, pitch, bitumen, mineral oil or paraffin or anycompound, product or residue of any such substance), hydrogensulphide poisoning, occupational skin diseases, lead poisoning, liverangiosarcoma, manganese poisoning, mercurial poisoning,mesothelioma, noise-induced deafness, occupational asthma,organophosphate poisoning, phosphorous poisoning, poisoning fromhalogen derivatives of hydrocarbon compounds, repetitive strain

    disorder of the upper limb, silicosis, toxic anaemia and toxic hepatitis.

    Criteria for Awards

    Applicants will be assessed on their lagging and leading indicators.

    Lagging indicators

    Applications will be evaluated only if the workplace:

    did not have any accident (including occupational disease) that had caused death orpermanent disablement, or that is classified as Dangerous Occurrence, in 2010/2011 and

    did not receive any stop work order in 2010/ 2011 and

    has an injury rate below 25% of the injury rate of its industry as given in the table 1below and

    did not have any reportable accident during its operation for the whole year of 2010(workplaces applying for WSHPA or SHARP, must be in operation for the full year in2010 to be eligible for this criteria),

    OR, the workplace have attained at least 1,500,000 man-hours worked without anyaccident, with at least 750,000 man-hours attained in 2010 (this criteria is alsoapplicable to any workplaces that ended its operation within 2010),

    OR, the workplace began its operation within 2010 and achieved 1,500,000 man-hours worked without any accident in 2010 (man-hours worked without any accidentin 2011 will not be taken into account).

    Leading indicators

    The workplace must approach an organisation providing auditing services (list oforganisations given in Part 6) to score its Safety and Health Management System. Toeligible for the Awards submission, it must achieve a Band III score in at least 70% of allelements in the checklist.

    Please note that achieving a Band III score in at least 70% of all elements in the checklist isthe minimum criteria, in the process of evaluation, the organiser will review all the scoresfrom Band I to Band IV.Therefore, companies must audit the full checklist.

    Should a company intend to indicate not-applicable in certain items within the checklist,evidence must be provided to prove that the item is indeed not applicable for the company.

    Special note on auditing organisation

    Please note that the same auditing organization can only provide audit service for acompany up to 2 consecutive years. Another auditing organisation must be appointed for the3rd year. This will commence from WSH Awards 2010, i.e. companies will need to re-appointa new auditing organisation from 2012.

    Disqualification before announcement of results

    The application may be disqualified if the following occurred in the workplace within 2011before the announcement of results:

    Any accident (include occupational diseases) that has caused death or permanent

    disablement. Any stop work order

    Any investigation, conviction or prosecution (including pending) of the workplacewith regards to matters concerning the workplace under the Workplace Safety andHealth Act or Workmens Compensation Act.

    Submission of falsified information to meet the Awards criteria

    How to apply for WSHPA using Occupational Safety and Health ManagementSystem (OSHMS) Audit Checklist

    1. Decide if your workplace should apply for WSHPA Award.

    You may use the checklist on page 8 to check if your workplace can meet theeligibility criteria. If it meets the eligibility criteria, you can do a self-score using Part 3of this document. Part 3 is an excel sheet which must be downloaded together withapplication document. Make use of the file, OSHMS.xls to help you calculate thetotal scores and percentage.

    If self-score is less than 70% of all elements within Band III, you may wish to improvebefore applying for the awards in 2011.

    If the workplace attains a score of 70% or more from your self-scoring of Part 3, youcan consider contacting one of the organisations which provide auditing services to

    evaluate your application. A list of approved organisations is also provided in Part 6.Please contact the organisation early.

    Please ensure accurate numbering of pages, use of hyperlinks and bookmarks inyour application to help the auditor find the evidences easily. Any information that iswrongly referenced will be assumed to be not available. No score will be awarded ifthe information could not be traced in the CD. Incomplete application will not beprocessed.

    Either you or your auditor may print Part 3. Do not print the supporting evidences.

    4. Please indicate your organisations name on the CD.

    5 The name of the auditor visiting the workplace must be in the list of auditors given inPart 6.

    6. The closing date is on 31 March 2011. The auditor must submit the completed Parts1 to 5 (hard copies) and the CD to the Workplace Safety and Health Council (WSHC)by that date.

    How to apply for WSHPA Award using ConSASS results

    1 Construction companies or worksites need not engage an organisation providingauditing services if they wish to use their ConSASS results which were audited from1 July 2010 till 31 Mar 2011. They must pass Band III of all the elements (i.e. scoredat least 70% of all elements within Band III). Organisations using ConSASS resultsneed not complete Part 3.

    2 You need to submit the following to WSHC:a. Hardcopies of:

    Parts 1, 2, and 4. Part 1 must be signed. Do not print the documentaryevidences. Part 4 must be completed.

    Photo copy of the ConSASS score card as given by the auditor

    b. CD containing: Part 1 Part 2

    Part 4 Safety and Health Management System manual of your organisation

    ConSASS audit checklist scored by your auditor all documentary evidences as required for the ConSASS audit

    Workplace visit for verification purposes

    All applications will be thoroughly assessed and verified by the organiser. This may alsoinclude a visit to the workplaces. The decision of the organiser is final. The organiser willnot accept any appeal.

    Terms and Conditions

    Application form must be correctly filled and signed. All supporting materials must beprovided in a CD. Late entries will not be accepted.

    All applicants are expected to ensure that all submitted documents are correct at thepoint of submission.

    To ensure fairness of Awards, amendments after submission will not be accepted.Application with inaccurate information will be disqualified.

    All winners are expected to work with the organiser to share their achievements with

    the industries following the announcement of the results. The organiser is not responsible for any loss or damage to entries or supporting

    material. The organiser will not return any entries or supporting material to the entrant.

    The organiser reserves the right to decline any entry by an applicant or organisationwho is being investigated for possible breach of Workplace Safety and Health Act orWork Injury Compensation Act (whether or not prosecution will be initiated) or anyother governmental regulations, or where the acceptance of the entry may bedetrimental to the objectives of the Workplace Safety & Health Performance Awards2011.

    The decision of the judges and/or organiser is final. The organiser will not accept

    any appeal.Notes for approved auditor

    1. The organisation applying for the Award will pay you for your services.

    2. Please note that the same auditing organisation can only provide audit service for acompany up to 2 consecutive years. Another auditing organisation must be appointed forthe 3rd year. This is to commence from WSH Awards 2010.

    5. Complete the columns labeled Results and Auditors Remark in Part 3. You must filland sign Part 5 before submitting your scores to WSHC. Parts 5 and 6 need not be in theCD.
