While templates such as the above one from DoHA or others from Divisions of General Practice are useful tools and act as a checklist to make sure all aspects of the descriptor are met (i.e. assessment and preparation). It is not necessary to use a template as long as you have adequate and contemporaneous medical records to reflect that the descriptor has been met.

services in a calendar year in groups of up to six services, although provision exists for patients to receive additional services where exceptional circumstances arise.
